Big controversy today - TBI patient with DI, Na 168. Night Dr given 5% glucose to bring it down... What do we think team? #emimcc #ansky #neurosky

Overloaded post-op patient in "deresuscitation phase". Boss wants to offload with albumin and furosemide together, then notices a "loop diuretic allergy", so opts for just the albumin. This makes no sense, right?! #nephsky #emimcc

Thanks guys! It was a TBI patient, on 1g IV BD for a week (prophylactically), so there were no compliance concerns. Was having odd ICP spikes which the neurosurgeons said could be seizures, but they never clinically seized at all. If it had been up to me I'd have added a second agent 🤷

Do we give loading levetiracetam because we're aiming for higher levels, or is it to get to therapeutic levels faster? Saw someone today "reload" a patient who had been on it for over a week, wondering if that makes sense? #emimcc

My current ICU has vitamin D supplementation as part of its admission bundle for all ICU patients.... #emimcc

Not particularly hypervolaemic by my reckoning, but yes they are over the critically ill part.

My current unit seems to use spironolactone specifically to treat hypernatraemia in ICU patients - thoughts? #emimcc

Hepatorenal syndrome (HRS-AKI) has cirrhosis in the criteria. So what do I call AKI in acute liver failure? Does the AKI have a different mechanism? #emimcc

Fury on the ICU today from the spinal surgeons as the spinal trauma patient has a MAP of 75 instead of 80. Let's review the evidence supporting their claims...

Genuinely at a teaching session where the speaker has 20-30ml/kg crystalloid in their initial management for haemorrhagic shock.

(Me on stage accepting my Froosy award) And I'd just like to thank the day shift, for giving everyone a billion litres of fluid every day, so I could along every night and sneak furosemide onto every drug chart while they sleep. You guys are the real heroes ❤️
