Identical prompts, different outputs every time. Guideline prediction is stochastic. This means single-prompt evaluations fundamentally miss the real performance distribution. We need large-sample, distribution-aware frameworks before deploying LLMs in clinical settings.

Key findings: - GPT-4.1 omitted guidelines in up to 46% of outputs; DeepSeek-V3 in up to 97% - Up to 9% of GPT-4.1 outputs hallucinated guidelines - Patient location (UK vs USA) drove citation rates from 0% to 78% - UK diabetes cases hit ~20% hallucination rates

#Equity in #digitalhealth tools is so reliant on different forms of data. Data on how people access #healthservices, how people engage with the #online world in general, data used to develop new #medical technologies, data used to measure meaningful #healthoutcomes in different population segments.
