Progression of Müllerian duct derivatives: 1) prostatic utricle 2) whatever I've got. Infantile uterus? It looks like it's got tubes 3) hypoplastic uterus 4) Persistent Müllerian Duct Syndrome uterus 5) normal fully developed uterus

And finally, another prostatic utricle, in a 6 y.o. Compared to mine, it's a relatively smaller (noting that they have to pull back some tissue to see it, where mine is plainly visible). So I guess I'm somewhere between a prostatic utricle and a hypoplastic uterus? I'll ask at my follow-up.

So, I went looking around for some references of hypoplastic uteruses and prostatic utricles to compare to my own laparoscopy, and I found a really good comparison in a case of Mosaic Turner's Syndrome (45X0/46XY). Turner's Syndrome first, my laparoscopy second:

Believe it or not Karl Marx did actually write about this specific thing.
Quote from The German Ideology by Karl Marx

"Further, the division of labour implies the contradiction between the interest of the separate individual or the individual family and the communal interest of all individuals who have intercourse with one another. And indeed, this communal interest does not exist merely in the imagination, as the “general interest,” but first of all in reality, as the mutual interdependence of the individuals among whom the labour is divided. And finally, the division of labour offers us the first example of how, as long as man remains in natural society, that is, as long as a cleavage exists between the particular and the common interest, as long, therefore, as activity is not voluntarily, but naturally, divided, man’s own deed becomes an alien power opposed to him, which enslaves him instead of being controlled by him. For as soon as the distribution of labour comes into being, each man has a particular, exclusive sphere of activity, which is forced upon him and from which he cannot escape. He is a hunter, a fisherman, a herdsman, or a critical critic, and must remain so if he does not want to lose his means of livelihood; while in communist society, where nobody has one exclusive sphere of activity but each can become accomplished in any branch he wishes, society regulates the general production and thus makes it possible for me to do one thing today and another tomorrow, to hunt in the morning, fish in the afternoon, rear cattle in the evening, criticise after dinner, just as I have a mind, without ever becoming hunter, fisherman, herdsman or critic. This fixation of social activity, this consolidation of what we ourselves produce into an objective power above us, growing out of our control, thwarting our expectations, bringing to naught our calculations, is one of the chief factors in historical development up till now."

The round ligaments of my uterus and some part of the spermatic cord meet up at a sort of Y junction near the deep inguinal ring and continue down the inguinal canal, but also stretch all the way back to the seminal vesicles, and it *isn't* the vasa deferentia. Any #medical #students want to help?

Might as well share this because it's pretty cool. So, I recently got a cystoscopy to check for intersex-related urinary tract irregularities. There's this divot in the prostate that *would* be the upper vagina and uterus if the Müllerian ducts developed. Here's a normal example, compared to mine:
A screenshot from a cystoscopy video, within the prostate. The verumontanum is visible in the foreground, fading into the background. The verumontanum is outlined in red, and the prostatic utricle is outlined in purple.A picture from my cystoscopy showing the verumontanum and "prostatic utricle". Both are significantly more prominent than in the normal example. The same screenshot as before, but the verumontanum is outlined in red and labelled, and the "prostatic utricle" is outlined in purple and labelled with a clarification that it is more accurately a persistent urogenital sinus of sorts, or a congenital urethrovaginal fistula, rather than what is normally called a "prostatic utricle" (implying a regression of the Müllerian that doesn't seem to apply in my case)

And here are my ovaries in the same frame as my testes. The left one was admittedly pretty difficult to find in the coronal view, but it's easy to see in sagittal view and then compare in the other planes. The second and fourth images are references from radiopaedia.org.
My right ovary, uterus, and left testicle in the coronal plane (vertical, parallel to the face). My uterus is again pretty clearly tilted sideways due to a large-ish fibroid, obscured a bit by the "Uterus" label and the red outline. The fibroid is the round shape at the bottom-right (my left) of the uterus in this view. My right ovary is nestled on top of my uterus. My left testicle is visible at the bottom of the image.A normal female pelvis reference from radiopaedia. It's almost identical to my own MRI- uterus in the center, right ovary visible nestled in on top.My own MRI in the coronal plane again. The left ovary is circled in orange- because of the high number of small follicles, it's a bit harder to see compared to the mottled surroundings, but there is a visible border around it. The sheer number of follicles and the way they're arranged more towards the outer edge of the ovary would be enough to say it has polycystic morphology. At the bottom of the image, both of my testes are also visible.Another reference from radiopaedia, showing a normal uterus and a polycystic left ovary in the coronal plane. The ovary does have a few larger follicles, but also many smaller ones. Not as many as mine, though.

Here's a comparison of my own uterus to a professional diagram and a reference cis woman's uterus on MRI. For some reason mine is somewhat lighter in appearance but otherwise it's pretty much the same. I also found my ovaries (the two normal ones and a potential third), similarly pretty standard.
My personal pelvis MRI, in the coronal plane (horizontal, perpendicular to the spine, rotated as if looking up from below), about 2 inches below my navel, with a small roughly triangular figure circled. There is a lighter "T"-shape inside, surrounded by darker curved shapes, which fade off towards the bottom-right of the area. This is my uterus. It's elevated to my left (the image's right) by what seems to be a fibroid, but is otherwise perfectly normal. This is very unusual, because I am a trans woman.A diagram of a uterus from Cleveland Clinic, with various anatomical features labelled including the Fallopian tubes, fimbriae of the tubes, ovaries, ovarian ligament, broad ligament body of the uterus, fundus of the uterus, cervix, endometrium, myometrium, and vagina. From EPOS, the Electronic Presentation Online System, the European Society of Radiology's image database. It shows "an axial T2 MRI image showing midcoronal plane of a normal uterus with the right way of measuring uterine wall thickness."

Like, this is basically what I have going on under the surface of the skin, a few snips and some stitches and this would be me if I wanted to preserve everything as much as possible. I know another (non-binary) intersex trans woman who does actually want that kind of bottom surgery.
An intersex anthropomorphic dog girl on her knees, body facing away from the viewer, with a hand spreading her upper thigh to show her vagina, scrotum, and penis.

Notably, her vagina is *just* her vagina, not including extra labia minor or an extra clitoris. This would be the appearance of some people with specific intersex variations if they had gotten a penile-and-scrotal preserving 'vulvaplasty' surgery, for lack of a better term.

In case there are any radiologists who run into this thread for some reason, here's what I'm looking at
Testicular ultrasound, of the left one. It's mostly normal, except there's a strip of some kind of brighter (more echogenic) tissue with some fluid-filled cavities near the surface. The brighter strip of tissue continues inwards, but without the larger fluid-filled cavities. There does seem to be one smaller one, potentially.

Oh *what*?! Yeah I do (visible in the ultrasound below). I didn't even know that was a thing. It feels like it comes from the ovaries themselves though? The ovarian part of my left ovotestis seems to be polycystic (see ultrasound again, it's on the middle of the right edge), which may factor in
A slice of my ultrasound, in horizontal (transverse) view. The top of my uterus is visible as a darker oval below my (full) bladder, the larger black oval. There are two extensions of my uterus visible in this slide- the oviducts/Fallopian tubes. My left ovary/ovotestis is also visible, just barely, on the right side of the image, on top of my left oviduct.

For reference, here's an actual report finding that trans women are actually more likely to be *disadvantaged*, funded by the IOC: www.forbes.com/sites/lindse...
Significant findings: Transgender women performed worse than cisgender women in tests measuring lower-body strength. Transgender women performed worse than cisgender women in tests measuring lung function. Transgender women had a higher percentage of fat mass, lower fat-free mass, and weaker handgrip strength compared to cisgender men. Transgender women's bone density was found to be equivalent to that of cisgender women, which is linked to muscle strength. There were no meaningful differences found between the two groups' hemoglobin profiles. Hemoglobin (Hb) plays a crucial role in athletic performance by facilitating improved oxygen delivery to muscles. Elite endurance athletes may exhibit up to a 40% higher level of Hb compared to untrained individuals. Moreover, heightened levels of Hb typically correlate with enhanced aerobic performance.

This is nothing. It just looks normal, maybe even cute in a way where I want to get a similar shirt now because I think it looks good. A feminine character simply having breasts does not sexualize that character. This is basic feminism stuff. Breasts are not inherently sexual. Especially like this.
A screenshot of a recording showing an Animal Crossing New Horizons mod that changes Isabelle's character model to have some fairly normal-sized breasts, completely covered up in a non-revealing way, such that her shirt just looks less flat.

My uterus, measured using a tool in a DICOM viewing program. 52.3mm by 30.6mm in the horizontal plane (i.e. a slice perpendicular to my spine from my front)Measurements for the ovary portion of my left pelvic ovotestis. 35.7mm by 22.7mm.A zoomed in shot of the left ovary with arrows pointing towards each follicle. There are a lot of small follicles all in a row, touching each other, a classical presentation of PCOS- except all the follicles are only on the side of the ovary facing the testicular portion, implying they're there due to testicular interference in follicular growth.Measurements for my right ovary, 34.1mm by 25.9mm. it's a little thicker and larger than my left ovary

Also, measurements for my uterus in this plane, 5cm by 3cm, a normal width and depthA zoomed-in view of my left pelvic gonad in the same pelvic ultrasound frame. It seems to be a bipolar ovotestis, with a larger ovary section and a smaller testicular section. Or maybe it's just PCOS, I don't know yet.Extreme closeup of my right pelvic gonad from a different frame of the ultrasound. It is just *structurally* difficult to get diagnosed with any intersex condition as an adult unless it causes fertility problems, cancer, or some sort of emergency. I'm still waiting to hear the radiologist report for an ultrasound I got last Tuesday. It might take a bit because it looks like this:
One of the frames from my ultrasound last week. It shows a squished uterus and vagina between a full bladder on top and my lower intestine on the bottom (also a tube running from the top of my vagina to my prostate)A frame from my ultrasound last week, showing my empty bladder and decompressed uterus and vagina, and much clearer view of the tube connecting my vagina to my prostateA closer view of my empty bladder and uterus, with an outline around my uterus, vagina, and the urogenital sinus connecting my vagina and prostate.A flipped and rotated version of the previous closeup with a different outline emphasizing the structure of my uterus. The endometrium is fairly visible, alongside the cervix. The vagina is more visible this way as well.

Look at the first two compared to some random references. If this doesn't get recognized as an obvious uterus and vagina then I don't know what to do at that point. I guess a second opinion and/or MRI?
My bladder ultrasound, in transverse view (left) and sagittal view (right). Below my bladder is a dark grey structure with a white line running through the middle of it (the uterus), exiting to another dark structure going off the bottom of the screen and bending back towards the right edge (the vagina). There is a tube splitting off from the vagina into the prostate, a black structure on the middle right of the image. An ultrasound scan of my bladder after emptying it, in transverse view (left) and sagittal view (right). My uterus is still visible, though there is a fan-shaped ultrasound shadow obscuring about a third of it (possibly indicating a uterine fibroid :/). The endometrium is very visible as a thick white line (I'm possibly going to be bleeding quite a bit soon...). The tube connecting my vagina to my prostate is even more visible in this frame, as a tube nearly running to the cervix about as thick as the prostatic urethra (also visible running from the bladder to the prostate). It can be seen that the vagina runs well past this tube and is directly beneath the prostate. The whole female reproductive tract is outlined with a thin red line to draw attention to it.A diagram and image of an ultrasound scan showing a uterus in sagittal viewAnother couple labeled ultrasound scans, the left showing a normal uterus and vagina, and the right showing a uterus with an intramural fibroid, describing how this type of fibroid appears scans

Something funny is that intersex advocates point out that the opposite happens when someone diagnosed early and "corrected" rejects the coerced assignment. There's no winning. From Hermaphrodite Logic, referencing two handbooks distributed by the Intersex Society of North America (ISNA) in the 00s:
Following from the earliest public arguments of the intersex movement, OII argued that the focus of intersex 'managenent' was the emotions of those challenged by intersex physiologies:

"The propaganda in this handbook repeatedly brushes aside the serious issues of transphobia and homophobia which many of us have experienced from our earliest...""...years and throughout life. The handbook is not about making intersexed children actually feel secure and comfortable with themselves but more about assuring the parents that their intersexed children will most likely NOT be transsexual or homosexual.
The whole reason for combining a lot of different conditions which have nothing in common medically under the umbrella term 'disorder of sex development' is not to treat real health conditions of intersexed children but to relieve the sufferings and anxieties that gender 'ambiguity' provokes within society."

For these advocates, it was adding insult to injury to have their variations rebranded as 'disorders'. OII argued that parents were told that 'DSDs' were most likely not linked to any irregularities in......gender identification, while doctors were issued with protocols for ensuring children's assigned sex and adult sex would align. The alternative, simply waiting to hear what children made of themselves, was spurned. Organization Intersex International argued that the framing of 'disorders of sex development' was an interlocking one with so-called 'gender identity disorder' (today largely replaced with a diagnosis of gender dysphoria):

"It is always the child who is disordered in these handbooks. If the intersex child rejects the arbitrary sex assignment, they have another disorder. What started out as a disorder of the child's sex has become a disorder of the child's gender . . . At all points of the treatment protocols, the child is always wrong and suffering from a...""...disorder."

In this way, 'DSDs' rearriculated longstanding clinical fears that intersex people would grow up to be homosexual. Together, these two manuals attempted to reassure parents and direct doctors in 'managing' children away from trans sexuality. Children would be subject to corrective procedures (surgical and hormonal) to minimise this new perceived risk. If they later opted to correct a wrongly assigned sex, they would be reclassified as suffering from another sinister acronym (from DAD to GID).

A sticker of a pregnancy test, with the trans flag on one side, and the text "Never stop trying" on the other. The test is positive.

Cis people will push for infant genital mutilation of intersex children because they don't want sex ambiguity *and* they don't want to deal with their child transitioning in *any* way later on. They'll enforce exclusive binary gender on babies born with ambiguous features. From Hermaphrodite Logic:
...medical profession was now at a loss when encountering a movement birthed from the Hermes of feminism and the Aphrodite of the gay movement. With the wisdom of consciousness-raising and barbed insights of '90s queer agitprop, the intersex movement left the clinical establishment repeating its mantras, even as it unravelled. One party had been forced to understand the other's terms all too well, and their wit was merciless.
The experience of this encounter left doctors disoriented as they witnessed the shaking of the foundations of sex and the refusal of the 'socialisation' model they had peddled. For their part, the intersex movement has reflected not only on their own sufferings, but the origins of these harms in the fearful psyches of those empowered overthem. As one participant in Hermaphrodites Speak! put it, "What's been taken is a very specific eroticism, a hermaphroditic eroticism that must really scare people, and really cause a great deal of anxiety."
Throughout the twentieth century, the divide between male and female had been shored up through sacrifice by those who had never volunteered. And from the 1990s to the present, those subjected to this sacrifice would speak back in force. While ostensibly replacing the 'optimum gender of rearing' model, the 'disorders of sex development' model retained the sense that professionalized guesswork on the part of medica would remain key to avoiding disaster. As philosopher Catherine Clune-Taylor has it, just like 'optimum gender of rearing' as a managerial framework, 'DSD aims at [poster's emphasis] securing a cisgendered future for the intersex patient, referring to a normalized trajectory of development across the lifespan in which multiple sexed, gendered, and sexual characteristics remain in "coherent" alignment.'
Their apparently modest gesture of ceding responsibility to families concealed an underlying continuity in the medics' aims. From the 2000s, the guiding priority would be to establish an outcome that prevented genital ambiguity where possible and [poster's emphasis] averted later transition between male and female. Medical authority would thus undergo an ostensible modernisation, without any basic principle being abandoned.

Hey all, I'm trying to help a trans woman who's been through some serious medical abuse get re-established in Los Angeles- currently she needs some emergency funds to get into a hotel. She's given me the OK to share her Venmo, she needs ~$150 more to stay somewhere for a few days and stabilize.
The Venmo account is Alicia Aimes (@AliciaIsADick)
