7/7 Next: Part 2C, on how MDBs can raise genuinely new equity capital — not just free up existing room. #BillionsToBalanceSheets #HealthFinancing #UHC #BlendedFinance

5/7 How: → Insurance on existing loans → Loan-swapping between regional banks → Guarantees from donor governments Billions unlocked this way — mostly for climate and infrastructure.

4/7 MDBs raise money easily. Their AAA credit rating is the asset worth protecting, and it shapes how much they can lend. So instead of raising new funds, they free up room to lend by shedding risk on loans already on their books.

P4H Network @p4hnetwork.bsky.social · 30/09/2026
3/7 @kchalkidou & Rahul Singhal propose multilateral development banks (MDBs) are the only actors with balance sheets deep enough, between them, to close a health financing gap this size.

2/7 Our last blog explored why most institutional capital - pension funds, insurance, central bank reserves, is hard to mobilise for health. This instalment takes it forward: if that capital can't move at scale, who can?

1/7 Development banks have freed up billions in new lending capacity in recent years. Almost none of it has gone to health. Part 2B of the P4H series on financing health systems in the age of the aid retreat. 🧵

(4/5) The obstacles aren't legal — they're practical: whose money it is, how fast it must be available, what it protects against, who has the expertise to judge the risk. Remittances add ~$56bn/year to sub-Saharan Africa — but that's household transfers, not an investable instrument.
100
P4H Network @p4hnetwork.bsky.social · 23/09/2026
(3/5) The most-cited fix: raise statutory ceilings on where pension funds can invest. In the largest markets, those ceilings are already generous and sit largely unused. Ghana permits up to 25% of pension assets in alternatives. Actual allocation: 0.58%.

(2/5) In this instalment, Kalipso Chalkidou & Rahul Singhal work through Africa's capital pools. Central bank reserves: held abroad. Most sovereign wealth funds: invested overseas. Set aside South Africa and the wealthier North African economies — roughly $250bn remains.

(1/5) Africa's institutional capital is routinely cited at $2 trillion. The figure within reach of the countries that need it most is closer to $250 billion. P4H's series on financing health in the age of the aid retreat asks where the money actually sits and who's best placed to move it. 🧵
100
P4H Network @p4hnetwork.bsky.social · 21/09/2026
(6/7) Step 5: Deal with risk from the start. Credit risk for start-up manufacturing in developing markets is genuinely hard to assess. Guarantees, first-loss facilities and transparent governance separate a fund that closes from a concept note that circulates for years.
100
P4H Network @p4hnetwork.bsky.social · 21/09/2026
(5/7) Step 4: Build in independence. How do you stop a politically anchored fund being steered by individual state interests? Decide upfront where the balance sits: political mandate, or duty to investors.
100
P4H Network @p4hnetwork.bsky.social · 21/09/2026
(4/7) Step 3: Match the money to the thing it's funding. A factory with years to revenue can't service debt due long before it earns, so lock in buyers through pooled procurement first, and the manufacturing becomes investable.
100
P4H Network @p4hnetwork.bsky.social · 21/09/2026
(3/7) Step 2: Raise the money in the right order, from the right people. Donors take first-loss with no strings, development banks absorb the middle risk, private capital takes the safest slice at market terms.

P4H Network @p4hnetwork.bsky.social · 21/09/2026
(2/7) A new @P4Hnetwork blog sets out 5 steps to design a blended finance mechanism for UHC. Step 1: Decide the rules before the design. Past funds spread too thin to matter, this one's betting on concentration over diversification, at a scale investors will actually notice.

(4/5) Wrong financing can distort priorities, entrench out-of-pocket spending, divert grant funding. This series explores what a practical, UHC-focused guide to blended finance for health systems could look like.
210
P4H Network @p4hnetwork.bsky.social · 18/09/2026
(3/5) The series asks: → How do we design blended finance for health without repeating past failures? → Where is the money held, and how much can genuinely move? → Which instruments have already unlocked capital at scale?
100
P4H Network @p4hnetwork.bsky.social · 18/09/2026
(2/5) Much of that conversation rests on large figures for capital that exists — not capital that can actually be invested. Over the coming weeks, @P4Hnetwork is publishing a 5-part series from WHO's Performance, Financing and Delivery dept on financing health in the "aid retreat" era.

P4H Network @p4hnetwork.bsky.social · 18/09/2026
(1/5) Where will the money for health actually come from? DAC funding fell >23% in 2025 - health funding potentially down 40%+. Governments are now scoping blended funds, debt swaps, and development bank balance sheets to reimagine health financing.

🇵🇭 PhilHealth aims to cut Filipinos’ out-of-pocket health spending from 44.7% to 25–30% by 2028, requiring higher gov’t health spending and bigger investments in primary care. Total health spending hit ₱1.4T in 2024, yet households still bear much of the cost. bit.ly/4ahTgYY #UHC #HealthFinancing
