![Multiple times since he’s been there, we have found feces on his tissue box, sheets, and even a blanket we brought in for him just the day before. We have also found his cannula on the floor and caught under furniture wheels.
He was not seen by the wound specialist who was there Thurs or Fri. Yesterday, Mon 8/24, was the first day the bandages on his heels were changed since he was discharged from the hospital. That’s 6 days without a change. We have confirmed that these are supposed to be changed daily. It became clear that at no point in the intervening time had the non-grippy socks he’d come in with been removed.
He was given a shower yesterday afternoon, which we were not aware was an option for him. It’s the first shower he’d had since his admission to the ER 5/29. After, he was left in the bed with no clothing besides a diaper, under a sheet and no blanket, and was very cold when [sis] arrived in the evening. It may be that we’d taken for granted the availability of grip-socks and clothing from other facilities but we were not told that he needed more, and the choice was instead made to simply leave him nearly naked.

![On Saturday 8/22, multiple serious issues arose.
[sis] had requested his transfer to a wheelchair to go outside, and the available O2 canister was not brought along. He was also not placed back on the bedside concentrator once he returned to the room.
The speech therapist who had seen [dad] previously did not seem to notice that he is missing most of his teeth and does not use dentures. I don’t know what other methodology is used to determine appropriate IDDSI texture levels, but based on his history of mechanical ventilation and issues with aspiration, on top of dental circumstances, we were surprised that his texture level was listed as 7.
After his outing in the wheelchair with [sis], where he went an extended period without O2 support, he attempted to eat dinner and choked on his first bite of chicken. In attempting to cough out the aspirated food, it was clear he was struggling with air intake because even his desperate coughing was shallow. [sis]’s initial concern about him looking like he’s not getting enough oxygen was brushed off ahead of him finally receiving help and getting the cannula reinserted.
[sis] and I separately confirmed his texture level was changed back to 5, minced and moist. On Sunday, she found he was again served level 7, regular texture.

![Additionally, we believe he may have been developing hypercapnia as his SpO2 has been consistently monitored at 96-98% throughout his hospitalization. We were shocked by a recent swift decline in cognitive capacity over a few short days, and the sudden development of a tremor, until the night nurse at [facility] let us know the ideal SpO2 is 88-92% for COPD patients. We are very grateful for that, but did still return to find that his O2 concentrator was being nudged higher than necessary. We are hoping that an appropriate SpO2 concentration going forward reverses symptoms, and more of his faculties return to him.
Due to the above ordeals, he’s been reduced to a whisper. He struggles to self-advocate. He is roomed with a person whose television is always on high volume. No one who calls him can hear what he’s saying over the din, and even when we are there with our ear up to his lips we struggle to understand his requests and can’t increase his comfort or reduce his pain. His hard-of-hearing wife can’t stay awake in the room because her hearing aids turn it into a constant fuzz. Days ago, when we asked if he could be switched rooms, we were told that because they were full, our only choice was to deal with it. Between all of this, we must insist that his contract be transferred to [former facility], whom we’ve spoken to about this, and has confirmed that they do have room to take him.
We were also told that contract transfers could in fact be done out of preference, counter to the impression [sis] was given by you, even before the contract had actually begun and we were still working through the original 20 day coverage. We have spoken to the administrative staff at [current facility] who were taken aback by the number and intensity of issues we'd seen.
