False dichotomy. I don't think the rest of the system should be profit-based. Creating a public utility for primary care is about building the baseline to protect patients & clinicians from market forces now. We can fight for broad financing reform while building public infrastructure on the ground.

Noha Aboelata, MD @nohaaboelatamd.bsky.social · 05/09/2026
Yes specialty care should be a right, but high-margin specialty care cannibalizes workforce & funding away from community-based health. Framing primary care as a public utility isolates/protects daily health infrastructure so prevention and local access can't be starved by the rest of the system.

Noha Aboelata, MD @nohaaboelatamd.bsky.social · 05/09/2026
Primary care as a public utility isn't at odds with single-payer which fixes financing; public utility fixes delivery & access. We need primary care as guaranteed, neighborhood-level public infrastructure so health systems stop starving primary care in favor of high-margin specialty services.

We must root these biases out before AI is layered into our systems! If we automate a broken foundation, we'll see our already-shameful disparities on steroids! I’m not going to pretend that making the necessary changes will be easy. But nothing will change if we don’t care. 3/

Clinical algorithm and hardware flaws are not just "tech glitches!" When medical devices perform poorly on darker skin, the real-world consequence is the systematic denial of treatment, benefits, and care for the people who need it most. 1/

This is in no way meant to diminish heroic efforts all over the US to keep people fed. But let's be clear, this is a crisis on top of multiple crises. It's painful to watch us repeat mistakes of the past 5 years while simultaneously dismantling all the systems that were built.

The SNAP crisis perfectly demonstrates the Panic-Neglect Cycle we do so well. Everywhere folks scrambling to plug the hole with short-term, unsustainable funds, creating budget instability and burnout across the entire safety net. We need a structural commitment to readiness.

Don't get me wrong, I frequently disagreed with things I heard in prior ACIP meetings, but at least we were all working with data and facts. Watching unqualified people make decisions for the country based on feelings, hunches, vibes & ideology is an excruciating waste of time.

"The ACIP is no longer a scientific committee. It is an ideology machine." I can't even bring myself to watch their meetings. Instead, I'm cheering for states and medical associations working to mitigate the damage and fill gaps. But the damage being done is immeasurable.

FINAL THOUGHT: When scientific process is not followed, the science that make the foundation for decisions is suspect and the decisions are therefore not valid. The ACIP S no longer scientific committee. It is an ideology machine. Istand by my justification for my resignation. Sending strength to the scientists being held hostage at CDC.

HHS initial cuts canceled nearly 700 CDC grants worth about $11 billion. Now, of the 5 states with the most canceled grants, 4 are republican-led: Texas, Georgia, Oklahoma, and Ohio.
Red S States Hit Hardest by CDC Grant Terminations
Before Democratic-led jurisd ictions filed lawsuits in Aprilto retain their Centers for Disease Control and Prevention grants, terminations more closely resembled the distribution of CDC funding across the nation. After injunctions were issued in May and June, red states lost a greater share of public health infrastructure funding and support.
