Interestingly I found that it wasn’t the pain of puncture that was bad, it was the gnawing aching of what I presume was arterial spasm afterwards. We do far fewer these days but definitely hurt enough patients that some young diabetics would admit avoiding ED due to anticipation of painful ABGs

Of course this is patient-dependent; and if human-resource-limited, will give real-time decision-making feedback for patient who is genuinely resuscitateable (VT/VF; younger patients). I think the take home is that being rural with limited resources doesn’t preclude good quality critical care.

Rural Resus tip no.6 from @emcrit.bsky.social at #SSEM25 - art line (under ultrasound guidance) is invaluable for arrests - you can use it for pulse checks, adjusted dose adrenaline, modification of mechanical CPR positioning… definitely far more reliable than pulse checks by hand #FOAMed #FOAMSky

Background Paediatric generalised convulsive status epilepticus (GCSE) are some thing we see a fair amount of in Virchester. Many are a result of febrile seizures, but as we are a tertiary centre we see the full range of causes presenting to PED. Seizures that fail to stop spontaneously within five minutes are much less likely. to terminate without treatment and may lead to progressive neuronal injury, worsening outcomes, and, in some cases, death.

The Japanese art of kintsugi recognises the value of a piece of pottery and in its damage, it is repaired, often with gold. Presentations are broken but should not be thrown in the trash. They can be repaired with kintsugi and returned to intended function and experiences of real value.
