8/ Future trials must fix the usual pitfalls: 🚩 Phenotype enrichment (LVH, high K gradient, volume overload) 🚩 Blinded adjudication 🚩Explicit competing-risk modeling 🚩Patient burden metrics #Nephsky

6/ Incremental dialysis may “preserve RRF,” but current evidence is observational, biased, and non‑adjudicated. Healthier patients are selected, not randomized. CV benefit is unproven. Only high‑RRF incident patients with strict monitoring fit the strategy

2/ High-volume HDF shows the strongest survival signal - but we must stay critical. Prior trials had center effects, adherence bias, and achieved-dose confounding. CONVINCE helps, but the mechanism remains opaque www.nephjc.com/news/convince

Dialysis adequacy MUST move beyond urea kinetics, and some of us have stated this for years. 🫀The dominant threat is cardiovascular stress: volumes, electrolytes, arrhythmia, LV load, inflammation... Kt/V is not enough #Nephsky academic.oup.com/ndt/advance-...

💡Nephrology inpatient volume increased 44%, outpacing other specialties ⏮️ Retrospective, 62 million admissions over 10 yrs 📍Patients became more complex: ⬆️multidisciplinary care, frailty& poorer primary care access 📍Complexity increased mainly independent of aging www.ajkd.org/article/S027...

4/5 Critical caveat: the MDR classification relies on breakpoints borrowed from related genera — no Psychrobacter-specific clinical standards exist. ⚠️Gene presence ≠ confirmed expression ⚠️The mcr-1 finding needs independent verification to rule out contamination

3/5 The strain inhibited 14 ESKAPE pathogens, including MRSA, K. pneumoniae, and P. aeruginosa. It carries biosynthetic genes for glycopeptides and bacitracin‑like products. A both antibiotic‑resistant and antimicrobial‑producing - ecologically coherent and medically intriguing.

2/5 SC65A.3 was isolated from Scarisoara Ice Cave's perennial ice block. Whole genome sequencing identified >100 antimicrobial resistance-associated genes, including mcr-1 (colistin), ampC (beta-lactams), and fluoroquinolone resistance determinants ⚠️No modern antibiotic exposure

7/ Participated in Kidney Tank. I’ll let you guess the hour; I'll just say we won. Tough brief, time crunch, tech issues, and jet lag, so we leaned on creativity and each other. Learned a lot about where social media in nephrology is heading #ISNWCN @theisn.org

Yes, agree. Some logistics to be considered. IN Europe, some iron forms are more expensive than EPO (Ferinject is the iron we give in my unit, so when I negotiate with the people in charge, I usually say that I'll cut down EPO) #NephJC
