The seminar clearly resonated with the audience. The presentation was followed by a lively discussion and many thoughtful questions: a reflection of how motivated the #CIPNet community is to rethink cancer research: What if the best cancer outcome is simply “No Cancer”?

The paper strongly echoed a central message of Dr. Castle’s seminar: if we all prefer “no cancer” to treating cancer after it develops, how do we build a research, healthcare, and societal system that truly prioritizes prevention?

NCI CIP-Net @nci-cipnet.org · 09/09/2026
Fineberg also outlines strategies for overcoming these barriers through better incentives, policy, communication, and systems that make prevention easier to adopt.
100
NCI CIP-Net @nci-cipnet.org · 09/09/2026
Its successes are often invisible, benefits may take years to emerge, financial incentives are not always aligned, and prevention is frequently held to a higher economic standard than treatment.
110
NCI CIP-Net @nci-cipnet.org · 09/09/2026
The paper explores a fundamental paradox of prevention: although virtually everyone values preventing disease, prevention can be remarkably difficult to implement and sustain in practice.

NCI CIP-Net @nci-cipnet.org · 09/09/2026
To conclude his seminar, Dr. Castle recommended Harvey V. Fineberg’s thought-provoking paper, “The Paradox of Disease Prevention: Celebrated in Principle, Resisted in Practice.”

Dr. Castle also recognized Roswell Park Comprehensive Cancer Center for its strong commitment to clinical trials and its continued support of DCP's cancer prevention research.

NCI CIP-Net @nci-cipnet.org · 09/09/2026
Under Dr. Castle's leadership, DCP continues to advance research across its major areas, including preventive agents, screening and early detection, and symptom science and prevention/management, with numerous programs and clinical research initiatives steadily moving the field forward.

Yet it is far too important to patients, families, and the communities we serve to be overlooked. This is why sustained public investment and a coordinated national effort are essential.
100
NCI CIP-Net @nci-cipnet.org · 09/09/2026
Dr. Castle discussed both the enormous potential and the unique challenges of cancer prevention. Prevention can be scientifically complex, require long-term investment and follow-up, and may not always provide the immediate financial incentives needed for the private sector to lead.

NCI CIP-Net @nci-cipnet.org · 09/09/2026
An onsite poll at the beginning of the seminar said it all: everyone wanted cancer to be prevented; no one chose to be diagnosed with cancer and then treated. It was a simple but powerful reminder of why prevention matters.

Dr. Philip Castle, Director of the NCI Division of Cancer Prevention (DCP), delivered an insightful and inspiring featured seminar on “No Cancer” Research, challenging us to think about what success in cancer research ultimately looks like: preventing cancer before it ever develops.

A powerful takeaway: patient perspectives need to be incorporated throughout cancer immunoprevention research:from risk communication and trial design to toxicity, quality of life, and dissemination of results.
000
NCI CIP-Net @nci-cipnet.org · 08/09/2026
The discussion also explored risk-benefit decisions, quality of life, financial burden, and access to understandable information so patients can make informed choices about prevention and treatment.

NCI CIP-Net @nci-cipnet.org · 08/09/2026
Patient advocates shared their cancer journeys and emphasized what prevention can mean to patients and their families, particularly for those with inherited cancer risk: the possibility of identifying risk earlier, preventing cancer before it develops, and potentially avoiding toxic treatments.
