Key messages: 1️⃣ Early identification and referral of patients with CKD through appropriate screening and timely specialist engagement is crucial. 2️⃣ Coordinated, team-based CKD care can improve education, navigation, surveillance, and management—supporting better transplant access.

Our key findings: 1) UK Patients on KRT are waitlisted more frequently and more quickly than Australia. 2) A higher proportion of UK KRT patients receive a transplant 3) Predictors of waitlisting are similar between UK/Aus 4) However, overall KRT mortality is higher in the UK than in Australia.
