Once PD has ensured ems personnel that there is nothing that can be used to harm yourself or others, we take it from there. That is now our scene, and from there we can make sure that you get the best quality treatment that we can provide.

As unfortunate as this is: PD will have to show up alongside EMS personnel. Their job is simple: check to see if you have anything that could be used to hurt yourself or others. You might say “I won’t hurt ems,” but the sad reality is that is HAS happened. It is absolutely nothing against you at all

The best thing to do would be to stay on the line with them until help arrives. If you do end up disconnecting, keep your ringer turned on and keep an eye out for any ‘no caller id’ contacts, as that’s them getting back in touch with you.

What I mean by this is please, by all means dial 988 if you are having a crisis. If EMS is needed immediately, you need to let the medical dispatch know exactly what is going on over the phone.

This really long thread may seem like there’s a lot to writing a detailed pcr, but with time and practice, writing one gets pretty easy. Moreso with approved acronyms. I.e. below is a mock pcr of a 40 y.o. male having an asthma attack.

Treatment and Transport: self explanatory here. What all did you do to treat the patient on scene? How was the patient transferred from the scene to your ambulance? Which hospital did you go to? Did anything change en route to the hospital?

Assessment: This is the meat and potatoes of your pcr. This is where you write down your full head to toe patient assessment. Everything you checked on the patient, you’ll write here.

History: What was going on before the patient’s complaint, what were they doing, were they eating or drinking anything, were they taking prescribed or unprescribed medication or substances, what’s their medical history, have they been treated for the same thing before?

Complaint: why did they call you? What is going on with them? How long has said thing been going on? What type of pain can they describe?

Description: simple as that. Describe the scene itself; what do you see, hear, smell? Same goes for the patient; do you see anything out of the ordinary, is the patient alert and oriented, what are they doing, are they standing or sitting or laying down, are there visible injuries, etc.

If you’re new to #ems, writing a pcr may seem intimidating or tedious, because you have to make sure everything is documented. And I mean everything. A good foundation that I was taught was the acronym DCHATT: Description, Complaint, History, Assessment, Treatment, and Transport.

Thread: For those that have fascinated about the idea of working #ems or healthcare in general, documentation is going to be paramount. A patient care report (or pcr for short) helps other healthcare providers know what’s going on with a patient. And heads up: you’ll be doing a lot of them.
