Agree that systemic not needed for cancer dx, but I worry about sampling bias - targeting visible lesion means targeting worst part and risk of over- grading. Feel at minimum hemi-biopsy necessary for proper grading and avoiding over-treatment of incorrectly graded G4+3 and G4+4.
