AI should be the swan song of the publish or perish evaluation model in academia but I suspect we will ride this ship deep down to the trenches of the ocean.

The really sad part is physicians won't meaningfully do nutrition care - they'll use their social capital to say conflicting, non-evidence-based stuff that gets clicks & then dietitians will have to fight an uphill battle correcting that knowledge & reprioritizing evidence based recs for 1/5 the pay

Sadly, the appeal of saying 'microbiome', listing off 'anti-inflammatory' foods and seeming 'integrative' by pushing supplements is far too appealing to too many physicians eager to build a following and a brand.

If you're not learning meaningfully about food composition, metabolism, dietary assessment, prospective cohorts/RCTs and their nuanced quirks in nutrition, you shouldn't be saying too much more than repeating basics from dietary or medical nutrition guidelines & referring to experts.

The RFKJr emphasis on medical nutrition edu -> now influencer MDs cosplaying harder than ever that they're nutrition experts pipeline is noticeable. Physicians have always been intrigued by the social capital they can get from nutrition(w little interest in the data) but I fear its set to get worse.

For folks in Nutrition, this is both unsurprising but sad. It's yet another blip in history after Michelle Obama's last efforts to change the food system that flopped. It risks real nutrition policy continuing to be seen as a politically charged untouchable, now w a side of gimmicky wellness trend.

Whether it was artificial dye-free candy, wellness rituals, or a technofascist embrace of oversold AI, MAHA was always doomed to fail. It's a movement whose mission fundamentally required more regulation but aligned with a deregulatory administration. Capture by COI was always the natural end.

We are moments away from a MAHA revisionist history project where the movement paints itself as righteous but overtaken by the commercial forces they claimed to be against. Never forget that MAHA was led by elite profiteers with a disdain for the scientific method & zero care for population health.

Online Now: Isoform usage dynamics in metabolic adaptation #trends #endocrinology #metabolism
dlvr.it
Isoform usage dynamics in metabolic adaptation
Metabolic adaptation is often framed as differential gene expression, yet nutrient state also remodels which transcript isoforms from a gene contribute to physiology. This review presents isoform usage as an organizing principle of endocrine and metabolic regulation: a layer that connects nutrient sensing to transcript architecture, protein function, tissue specialization, and disease vulnerability. We synthesize evidence that fasting, feeding, insulin, and metabolite availability reshape isoform outputs across metabolic organs independently of total mRNA abundance. We then integrate functional examples, nutrient-responsive RNA-processing mechanisms, human genetic and disease data, and emerging RNA-targeted interventions. These findings suggest that metabolic disease reflects not only dysregulated gene expression but also failure to maintain the appropriate isoform state required for adaptive physiology.

It is weird being early career stuck in a system that's resigning itself to irrelevancy. In part I think it's refusing to change, but in other parts, I think folks at the top genuinely are stuck in a 40yr old model. We desperately need fresher leaders before we publish/perish our way into the sun.

You can't say 'trust the science' to a public that is regularly questioning the value of your existence and your outputs, while you also sit by as existential threats (from R&R to AI) sit at your doorstep and you barely let out a peep apart from toothless policies that change no incentives.

The peer reviewer crisis is only being made worse by the peer reviewers agreeing to review but turning in AI slop as the final review. Have seen it as an editor and had it happen to me. Unfortunately, the fanfare hasn't focused on first principles/ translatable components and has more deified the pattern, despite its limited cultural adaptability. Theres probably nothing all that uniquely special about the MedDiet (eg many high polyphenol options across traditional diets)
