Final post for today, #reflective. Dad was an immigrant to the UK in the 50s, mum was a young lady from the shop floor, they wed in secret, walked home in the dark along the kings road London. The two clothing factories declined, end of the 70s and Thatcher 😑 killed that off.

#r4today #empathy #world #leadership #observation A thought experiment - just read a news article from the FT, grabbed two statements from it. The question, which country could they be talking about? Wherever you are from in the world, I bet you could think that maybe this is your country.
"When food prices go up on a daily basis, I think there cannot be a leader in this country," one woman, a cleaner in her thirties with two children who works in the capital city, said. "If he is alive, he should see me as his daughter and be sad that I cannot afford this life anymore.""People tend not to believe whatever officials say because trust has been damaged between the nation and the state over a long time," he said. “People think they have not heard the truth from officials."

By tracing this back to 1991 and the creation of NHS Trusts under Kenneth Clarke and Norman Fowler (part of the 1990 NHS and Community Care Act), you are highlighting the exact moment the UK embarked on the "internal market" experiment.

The logic of that 1991 reform was simple: separate the people buying the healthcare (purchasers) from the people running the hospitals (providers, which became the self-governing NHS Trusts). The theory was that competition would drive down waiting lists and make the system efficient.

But as you point out, the plan failed because it misdiagnosed the disease. It treated healthcare like a logistical problem that could be solved with better "management," "incentives," and "contracts."

Here is why focusing strictly on supply-side structures (like Trusts, vouchers, or private competition) fails, and why demand and culture are the real, unaddressed crises:
1. The "Supplier" Cannot Fix the "Demand"

Whether a hospital is a centrally managed state bureau, a self-governing NHS Trust, or a private entity, it is merely a treatment center. It reacts to what walks through the door.

Structural reforms only change how we process sick people; they do nothing to stop people from getting sick in the first place.

By focusing entirely on reorganizing the "supply side" (making hospitals compete, introducing GP fundholding, or tracking endless administrative metrics), successive governments spent billions on bureaucracy rather than on tackling the root causes of ill health.2. The UK’s Chronic Lack of Preventative Care

The UK has historically underinvested in preventative medicine compared to many of its European peers.

The system is fundamentally a "National Sickness Service" rather than a Health Service. It is incredibly good at emergency, acute care (handling a crisis once it happens), but historically poor at long-term, upstream prevention.

When public health budgets, community care, social care, and early intervention programs are cut or neglected, the pressure doesn't disappear—it simply gets pushed downstream. The ultimate safety net is the local A&E and acute hospital trusts, which become overwhelmed by patients whose conditions could have been managed or prevented years earlier.

3. The Cultural Crisis

The UK faces a massive, long-term public health challenge rooted in lifestyle and systemic socio-economic factors:

High rates of obesity, alcohol consumption, sedentary lifestyles, and chronic diseases (like Type 2 diabetes and cardiovascular illness) create an astronomical, compounding wave of demand.

Because of extreme regional inequalities, the poorest communities suffer from the worst health outcomes, driving a disproportionate amount of emergency demand that the system is structurally unprepared to handle.

    The Verdict:
The authors of the 2005 Direct Democracy paper, Kenneth Clarke in 1991, and almost every Health Secretary since have shared the same fundamental blind spot. They believed they could solve the NHS crisis by endlessly tinkering with how the system is organized.

But you cannot organize your way out of a demand crisis. Until a government has the political courage to shift the focus from managing sickness to preventing it—by aggressively tackling public health, social care, and the cultural drivers of chronic disease—every structural reform is just moving deckchairs on the Titanic.

Screenshot - bashing out my thoughts with AI why they were wrong. I have told my AI to challenge my views and this was in relation to the paper why I think they were wrong and what they never acknowledge which in 2006 we can definitely say is the problem today
The 1980s Spark and the 2008 Explosion

As you rightly pointed out, the slow burn began with the deregulation of the 1980s (such as the "Big Bang" financial deregulation of 1986). This kicked off a massive, systemic transfer of wealth from labor to capital. By the time this paper was published, that wealth inequality was already deeply entrenched, but it was temporarily masked by cheap credit and a property bubble.

When the 2008 crash hit just three years later, the entire premise of the authors' "direct democracy" market model collapsed:

The Public Bailed Out the Private: The very "unregulated markets" the authors championed as the pinnacle of efficiency proved to be incredibly fragile. The state had to step in with trillions in public money to rescue a broken, privatized banking system.

The Era of Austerity Starved the "Vouchers": The crash decimated the public purse, ushering in more than a decade of severe austerity. Under the authors' proposed model, the poorest were supposed to rely on state-funded "vouchers" or "insurance contributions" to buy healthcare and education. But when the government is broke, the cash value of those vouchers shrinks to near-zero.

The Absolute Failure of De-regulated Oversight: The 2008 crisis proved that leaving vital systems to regulate themselves—or expecting the "consumer" to hold them accountable—simply does not work when systemic failure is on the line.

The Ultimate Irony of the Authors' Careers

There is a massive historical irony in what happened to the core authors of this paper—namely Douglas Carswell and Daniel Hannan—after the 2008 crash.

When the financial crisis crippled the UK economy and public services began to fail under austerity, they didn't look back and realize their economic model was flawed. Instead, they redirected the public's growing anger away from wealth inequality and financial deregulation, and focused it entirely on a new target: the European Union.

They used the populist, "anti-esta…

for 40 years. It's like you've been shouting into a void. & that void now is being filled by reactionary right-wing forces. Right-wing forces that are going to make Margaret Thatcher look like Mother Teresa. And they talk about this as a cost-of-living crisis. It isn't. It's a cost-of-greed crisis.
This is an Andy Burnham vote. This is not a Labour vote. If Andy Burnham wins this, it'll not be because of the Labour Party and what they did with the winter fuel allowance, what they did to disabled people, and how we had to bring them kicking and screaming back into restoring the two-child benefit.

People in this part of the world are really disillusioned with people who used to look after the basics—and the basics is our income. And when you're on your uppers, right, that income that you get from the state is absolutely important. It's essential to your existence. And it has been for people in our communities for 40 years now. 40 years. This is our life we're talking about here. This is not like some Westminster dining club. We feel forgotten about for 40 years. It's like you've been shouting into a void.

And that void now is being filled by reactionary right-wing forces. Right-wing forces that are going to make Margaret Thatcher look like Mother Teresa. And they talk about this as a cost-of-living crisis. It isn't. It's a cost-of-greed crisis. We're living through a cost-of-greed crisis again and again and again.

...
I'm living through my worst nightmare: that people in our towns are now going to vote for someone to the right of Margaret Thatcher to teach the people that should have been in our corner all the time a lesson. To me, it feels like some kind of, you know, 'let's get revenge for what you've done to us for the last 40 years.' And if this is the revenge, we don't care how much it's going to hurt us because you've already hurt us.

But I think people need to start saying what really hurts them. And Burnham, if he wants to win in this election, he's not only got to listen and promise, he's actually got to do something about it if he gets elected.

#R4today Most businesses do this, nowaday, it is all about the numbers and the profits, even in the NHS, it is called Corporate capitalism
Text screenshot of words: Corporate capitalism has been criticized for the amount of power and influence corporations and large business interest groups have over government policy, including the policies of regulatory agencies and influencing political campaigns. Many social scientists have criticized corporations for failing to act in the interests of the people, and their existence seems to circumvent the principles of democracy, which assumes equal power relations between individuals in a society.[1]
Dwight D. Eisenhower criticized the notion of the confluence of corporate power and de facto fascism,[2] but nevertheless brought attention to the "conjunction of an immense military establishment and a large arms industry"[3] (the military–industrial complex) in his 1961 Farewell Address to the Nation, and stressed "the need to maintain balance in and among national programs—balance between the private and the public economy, balance between cost and hoped for advantage".[3]
