Straight men rarely struggle to understand consent when they picture themselves receiving unwanted sexual attention in a gay bar. Move the setting to a fraternity house or dorm room, though, and suddenly people start inventing exceptions. Walking through a door is not consent, drinking is not consent, flirting is not consent, and nobody gets sexual access to another person simply for being there.

September 23 is Bisexual Visibility Day, often called Celebrate Bisexuality Day or Bisexual Pride Day, and it is a day built around something that should sound ridiculously simple by now: bisexual people deserve to be seen as bisexual people.

That is one of medicine’s greatest victories, but survival has created a new set of challenges: cardiovascular disease, cancer, bone loss, cognitive changes, polypharmacy, poverty, isolation, survivor grief, and health systems still learning how to care for an aging HIV population. People fought for the chance to live longer. Now the measure of progress is whether those added years can be lived with health, stability, connection, and dignity.
