It's like a list of all the things that go wrong in Internet policy. Here is one that is very fundamental and recognisable, for example, in the online space among others
THE TRAP: SUCCESS TO THE SUCCESSFUL
If the winners of a competition are systematically rewarded with
the means to win again, a reinforcing feedback loop is created by
which, if it is allowed to proceed uninhibited, the winners eventu-
ally take all, while the losers are eliminated.
THE WAY OUT
Diversification, which allows those who are losing the competition
to get out of that game and start another one; strict limitation on
the fraction of the pie any one winner may win (antitrust laws);
policies that level the playing field, removing some of the advan-
tage of the strongest players or increasing the advantage of the
weakest; policies that devise rewards for success that do not bias
the next round of competition

Here is how we might view the Online Harms policy system. Hopefully it is entirely clear that online harms policies are nowhere near the engine producing them, and that the policies are self-reinforcing in the face of failure

New Health sec James Murray MP wants the NHS to "put our feet to the floor on reform". Isn't speeding up and taking risks how people end up in A&E? And isn't it how the NHS ended up with #Palantir?
What we have done in the first 2 years, on the back of record investment, is to make improvements while starting to modernise the NHS. 

And now that we have stabilised the system, my job is to work with you not only to maintain and extend those improvements in performance, but also to put our feet to the floor on reform.  

What I am bringing to this role is not a change of course but a change of gear. 

I am determined that we accelerate our fundamental transformation and modernisation of the NHS by making sure that we make the fullest possible use of technology, digitisation and AI.

This word ‘system’ has become so much a part of our twenty-first century vocabulary, as in ‘the transport system’ (when it breaks down), ‘the social security system’ (ditto), a ‘stereo system’ etc., that we probably take its use for granted, and do not consider some of the implications of using it. Really to think in terms of systems is not necessarily so easy, but is an essential part of our outlook if we are to develop our world in a sustainable manner.

A classic example arose from the series of rail crashes in England in the first years of this century. Tragically, several people were killed, and the obvious ‘cause’ was problems with the rails. To avoid further loss of life, draconian speed limits were imposed on the trains and repairs to the tracks instigated. This no doubt reduced the chances of further rail accidents, but in the process, persuaded many people to abandon rail travel in favour of their cars. Given that the probability of an accident per kilometre travelled is a couple of orders of magnitude larger for car travel than rail travel, the decisions taken about the railways may actually have increased the number of travel-related deaths and injuries, rather than reduced them. A decision taken about the safety of the railway system may well have had completely the opposite effect to that intended when considered in relation to the wider transport system.

A strengthened commitment of the State

In line with the recent directives communicated by the Prime Minister, in particular the circulars on numerical public control and the generalization of the video conferencing tool "Visio", the seminar made it possible to set a clear objective: to reduce the non-European digital dependencies of the State.

Several concrete first steps already illustrate this ambition:

    Regarding the evolution of the workstation, DINUM announces its release of Windows for the benefit of Linux operating system desktops.
    Regarding the migration to sovereign solutions, the Caisse nationale d'Assurance maladie announced a few days ago the migration of its 80 000 agents to tools of the inter-ministerial digital base (Tchap, Visio and FranceTransfer for the transfer of documents).
    Last month, the Government announced the migration of the health data platform to a trusted solution by the end of 2026.

The government has today (8 September) announced urgent action to
toughen the Online Safety Act by putting stricter legal requirements on tech
companies to hunt down and remove material that encourages or assists
serious self-harm, before it can destroy lives and tear families apart.

While platforms already have to take specific steps to protect children from this dangerous self-harm content, the government recognises that adults battling mental health challenges are equally at risk from exposure to material that could trigger a mental health crisis or worse.

This kind of exchange could easily be avoided IMO, just by simply stating that the OSA needs "wholesale revision" or even, "reform", rather than "scrapping" it. #nigelfarage walked into a trap here, and it was avoidable. #onlinesafetyact

Extraction of online information following seizure of electronic devices
(1) Where an electronic device has been lawfully seized, a senior officer may
authorise an enforcement officer to extract information accessible by means
of one or more online accounts which were accessed by means of the device
before it was seized.
(2) A senior officer may give an authorisation under subsection (1) only if satisfied
that there are reasonable grounds to believe that—
(a) the information mentioned in subsection (1) includes information that
is relevant to a reasonable line of enquiry which is being, or is to be
pursued, by an enforcement officer for one or more relevant purposes,
and
(b) it is not reasonably practicable to obtain that information by other means.
