Blue Origin announced late today that the FAA accepted the company's investigation into the NG-3 New Glenn launch failure last month. FAA statement:
The FAA-required investigation of the Blue Origin New Glenn-3 mishap that occurred on April 20 is closed. There were no public injuries or public property damage.

The FAA oversaw and accepted the findings of the Blue Origin-led investigation. The final mishap report identified the direct cause of the mishap as a cryogenic leak that froze a hydraulic line and led to a thrust anomaly during the second stage engine burn.

Blue Origin identified nine corrective actions to prevent reoccurrence of the event. The FAA will verify that Blue Origin implements corrective actions prior to the launch of the next New Glenn mission.

The Blue Origin New Glenn vehicle is authorized to return to flight provided all other licensing requirements are met. Contact Blue Origin for more information.

FAA has authorized SpaceX to resume Falcon 9 launches after an upper stage deorbit burn failure Monday.
FAA Authorizes SpaceX Falcon 9 Return to Flight

The FAA-required investigation of the SpaceX Starlink 17-32 mishap that occurred in space on Feb. 2 is closed. There were no public injuries or damage to public property.

The FAA oversaw and accepted the findings of the SpaceX-led investigation. The final mishap report cites the probable root cause was the Falcon 9 stage 2 engine's failure to ignite prior to the deorbit burn. SpaceX identified technical and organizational preventative measures to avoid a reoccurrence of the event. The Falcon 9 vehicle is authorized to return to flight.

Wow: Blue Origin says it will pause New Shepard flights for at least two years to accelerate work on human lunar spaceflight:
Blue Origin to Pause New Shepard Flights for No Less Than Two Years

Resources will be redirected to further accelerate lunar human flight program

Blue Origin today announced it will pause its New Shepard flights and shift resources to further accelerate development of the company's human lunar capabilities. The decision reflects Blue Origin's commitment to the nation's goal of returning to the Moon and establishing a permanent, sustained lunar presence.

New Shepard is the first reusable spaceflight system to vertically land and has flown 38 times and carried 98 humans above the Kármán line to date. New Shepard has launched more than 200 scientific and research payloads from students, academia, research organizations, and NASA. This consistent and reliable performance, combined with an exceptional customer experience, has resulted in a multi-year customer backlog.

They discussed NASA’s role in maintaining America’s competitive edge over China and other foreign adversaries in space exploration by returning man to the moon to acquire critical minerals and resources vital to national security and continuing to bolster NASA’s Lyndon B. Johnson Space Center (JSC) in Houston, Texas, as the human space flight center of excellence and innovation. Mr. Isaacman committed to follow Sen. Cornyn’s provision of the One Big Beautiful Bill Act, now law, to move the Space Shuttle Discovery in one piece from Virginia to its rightful home in Space City. Please see photo attached and below.
