An authoritarian government can simply declare who is a citizen and who isn't. If you're an "enemy" you can have your citizenship stripped. And what are you gonna do about it? Those who believe they're protected because they are citizens are not thinking this through. (See Reich Citizenship Law)

I will say this, and I realize it's anecdotal: My husband was diagnosed with cancer the day he went on Medicare. Medicare, along with a Plan G supplement (total premiums about $320 a month), covered EVERYTHING, with the exception of the Medicare deductible.
