A bit overwhelmed by my visit to the Taj Mahal this morning with my daughter. Built in memory of Mumtaz Mahal, the wife of Emperor Shah Jahan, who died giving birth to their 14th child in 1631. Poignant — and even more beautiful than I could have imagined.

There is much to be welcomed about the Amos report - but its conclusions on the influence of ‘normal birth’ ideology on maternity safety are not supported by wider evidence, offer dangerous false assurance and must not be relied upon. www.hsj.co.uk/patient-safe...

The Amos review’s conclusions on ‘normal birth ideology’ are not credible
The Amos review's conclusions on maternity safety contradict four previous major investigations, raising serious questions about the evidence underpinning national policy

We have rightly moved away from a culture of individual blame in patient safety. But in doing so, have we created a dangerous blind spot around individual responsibility and accountability? I’m aware this may be a controversial piece! www.hsj.co.uk/patient-safe...

Blaming individuals for care failures is sometimes the right response
The Great Ormond Street scandal exposes a growing imbalance in NHS patient safety policy. In moving away from blame, the system has also lost sight of individual competence, leadership responsibility, and the non-negotiables needed to prevent serious harm

🚨 INVESTIGATION: Britain's maternity crisis has deeper roots, extending into the universities trusted to train the next generation of midwives. A majority are still promoting a normal birth ideology while women are more complex than ever: READ: www.thetimes.com/article/b4fe...

One of the best things in the Hillsborough Law Bill is not the duty of candour on public officials, as welcome as that is, but news that bereaved families will have publicly funded legal representation at inquests. Finally an equality of arms that's been lacking for so long
