In PHAN we’re about to go through an expert review to determine, how we, as a small non-profit, can most effectively reduce the spread of airborne disease. If you have creative, high leverage ideas for us to pursue, please submit a proposal for us to evaluate alongside our own ideas.

AI is evolving too quickly for an annual report to suffice. To help policymakers keep pace, we're introducing the first Key Update to the International AI Safety Report. 🧵⬇️ (1/10)

Oh, actually it was ~3/15. But all of my own time. I then handed things over to my co-author who did a ~12 minute talk on GANs

@publichealthaction.bsky.social now has a blog. @juliadoubleday.bsky.social has written a guest post about her experience living with long COVID.

Another more specific way people can help: does anyone know if it’s safe to take betahistine if I have MCAS? And does taking antihistamines make betahistine ineffective?

No, I’ve never been a heavy headphone user and always been careful about my noise exposure (before getting ear disease). You can see in my profile pic I even wear “cat ears” when biking

I’ve seen some web pages that say betahistine can lead to increased histamine synthesis. Do you know if that’s accurate? My ears have responded well to migraine treatment so it would make sense for them to respond badly to more histamine

I tried betahistine for about 4 days and it seemed to make things worse. At the time I had not been off diuretics too long and my ears were still fairly stable. After a few days on betahistine I got woken up by tearing / popping sounds in my right ear + new pure tone tinnitus

Eagles syndrome: I don’t have any pain or any trouble swallowing or turning my head. I’ve had a CB-CT scan of my head and the radiologist didn’t notice any Eagles syndrome

Thanks. Yeah, I wear compression socks + tights + abdominal binder. I had a vascular ultrasound done of my neck and didn’t find anything wrong, not sure if that rules out everything.

As far as I know it’s the amount of sodium that reaches the inner ear (and how much that level fluctuates) that matters, not the delivery method.

Thanks. Creatine is probably problematic for Meniere’s because of fluid retention, but there are other mitochondrial supplements I can try.

My pulse goes up from like 43-50 range if I’m flat to 47-55 range propped up. I feel like my pulse would probably go higher if there was a cerebral O2 problem, but also I wake up a ton. I don’t know if the wakes are COVID induced primary insomnia or if they’re driven by low O2.

That’s an interesting idea. My wrist SpO2 is good. My understanding is ear lobe SpO2 isn’t reliable. I wish I had a way to measure head SpO2.

A few ideas for leads would be if there is anyone with experience treating Meniere’s with amiloride or fludrocortisone, or anyone with experience treating POTS patients who have to take diuretics for more common reasons than Meniere’s.

My Meniere’s doctors are great but they don’t know a lot about POTS or POTS-compatible treatments. I’ve had less access to POTS doctors and they don’t know much about Meniere’s. I’m really struggling to find a way to manage both. Any help finding someone who knows what to do is greatly appreciated.

Many POTS patients benefit from eating several thousand milligrams of sodium per day but for Meniere’s I need to keep my sodium under 1500 mg per day. I get noticeable damage if I screw up even one meal.

Ian Goodfellow @ian-goodfellow.bsky.social · 24/11/2024
With unstable Meniere’s, my ears pressurize and lose hearing if I lower my head, so I have to sleep propped up quite high, but this is horrible for POTS, since my body struggles to get blood into my head. Obviously this is bad for my sleep generally and causing a wider downward spiral.
1112
Ian Goodfellow @ian-goodfellow.bsky.social · 24/11/2024
The huge problem for me is that POTS and Meniere’s have basically opposite management requirements. Diuretics are very bad for POTS so I can’t take a high enough dose to keep my Meniere’s stable

Ian Goodfellow @ian-goodfellow.bsky.social · 24/11/2024
This year I unfortunately got POTS from COVID. POTS is a failure of the body to regulate blood flow correctly, the heart has to beat extra fast to get blood to reach the head when upright. It’s much more complicated than that but that’s the defining symptom.

My early 2023 hearing loss was diagnosed as endolymphatic hydrops / the cochlear only version of Ménière’s disease. It causes progressive hearing loss, feelings of pressure in the ear, and tinnitus. There is no cure, but it’s been stable with low salt diet, a diuretic, and an anti-migraine drug.

In early 2023 I started losing hearing suddenly and got the best help by posting on Twitter, so I’m trying that strategy again

Posting a call for help: does anyone know of a good way to simultaneously treat both POTS and Ménière’s disease? Please contact me if you’re either a clinician with experience doing this or a patient who has found a good solution. Context in thread
