Hey, I'd give it a 6/10 1. Above the fold copy -> instead of Urgency and scarcity repeatingI'd tell why is it different and give a nice subheading 2. In the reserve your spot CTA -> the first question apart from personal info seems to be about you. (don't wanna mess up comments so DMing the rest)

Very strong 9/10 1. Starts right off the bat, giving a trial also, strong plus 2. Goes through pain points 3. and then swiftly shows the testimonial as well 4. The example could be better, show readability from 52% to something 60+ (rest sent in DMs, don't wanna mess the comments)

So, make sure you make it enticing for end users as well. Future idea - gamify this. The more feedback the customer gives, they collect coins, and they'll get discount. (The feedback should be approved by the developer tho) But don't go ahead making a feature before fixing this and validating

The gif was also very fast so it was a bit hard to understand. I only got to understand it when you said it's for visitor feedback and dev design collab. It's a Nice app, and a good competitior to usual "Request a feature" thing everyone has But you need to fix the positioning a bit.
