You don't need to become a fitness influencer. You need to: Not die at 52. See your kids get married. Play with your grandchildren. Not become a burden on your family. That's the goal. Everything else ,-the walks, the diet, the medicine is just the price of admission. Pay it willingly.

Most people's idea of "health checkup": Go to a fancy lab. Get 87 tests done. Receive a thick report. File it without reading. Feel responsible for 6 months. Repeat. No understanding of results. No action taken. No follow-up. This isn't healthcare. It's expensive theater.

"I play cricket on Sundays. That's my exercise." No. That's recreation. You sit 6 days a week. You play 2 hours on the 7th. You eat pakoras after the match. You call yourself "active." Your heart doesn't count Sundays differently. It counts the 160 hours you sat still.

Same patient: ₹2 lakh on daughter's wedding jewelry. Won't spend ₹200 on a monthly lipid panel. ₹50,000 on a new phone every year. Won't spend ₹500 on BP medication monthly. ₹5,000 on a single dinner. "Gym membership is too expensive." We don't have a money problem. We have a priority problem.

When a 50-year-old man dies of a heart attack, people say "achanak ho gaya." It wasn't sudden. It was 20 years of smoking. 15 years of uncontrolled sugar. 10 years of ignored cholesterol. 5 years of skipped medicines. The death was sudden. The dying was slow. His family just wasn't watching.

The truth no one wants to hear: Your heart disease is not bad luck. Your diabetes is not genetics. Your hypertension is not "just stress." It's decades of choices compounding. The good news? It can reverse the same way. One choice at a time. But you have to start. Today. Not after Holi

Trust in medicine is at an all-time low. Wellness gurus know this. They exploit it. "Big pharma is lying." "Doctors don't know nutrition." "The system wants you sick." And while you're busy distrusting the cardiologist— The guy selling you ashwagandha gummies is laughing to the bank

Everyone wants a hack. "What's the one superfood for heart health?" "Which supplement reverses plaque?" "What's the secret doctors don't tell you?" There's no secret. There's no hack. Eat clean. Move daily. Take your medicines. Follow up regularly. That's it. It's just not sellable.

Interesting pattern: people who start walking don't stop at walking. They start caring about what they eat. How they sleep. How they spend their time. One act of self-respect seems to demand another. Like your body remembers what it feels like to be cared for and refuses to settle for less.

One patient changed everything. 52 years old. Diabetic. Hypertensive. First heart attack. I told him straight: "You have maybe one more chance. Use it." He did Lost 18 kg in a year. Walks 10,000 steps daily. LDL now 70. Off half his medications It's possible. Most just choose not to

"I'm too old to start exercising." I have 70 yrs olds doing cardiac rehab after bypass surgery. "I'm too young to worry about this." I have 35 yrs-olds with three blocked arteries. Age is not a reason. It's an excuse dressed up as wisdom. Start now. Your age is irrelevant

Dr Shailesh Singh @heartdocshailesh.bsky.social · 07/01/2026
I ask every new patient: "What should your LDL be?" 90% don't know. They've had blood tests for 15 years. They've seen their reports. They've filed them carefully. But they never learned what the numbers should be. You can't hit a target you've never seen

Me: "Your LDL is 180. We need to start a statin." Patient: "I don't want chemicals. I'll control it with diet." Me: "Okay. See you in 3 months." 3 months later: LDL: 185. Diet: Same. Exercise: None. Patient: "Maybe I need something stronger than diet." We lost 3 months. The plaque didn't wait.

"Red wine is good for the heart." This might be the most successful lie in health history. One flawed study. Decades of repetition. Exposed. There is no safe amount of alcohol for your heart. The French Paradox was never a paradox. It was bad science and good marketing.

Doctors are trained to be polite. "Try to reduce smoking." "Maybe cut back on sweets." "Consider some lifestyle modifications." What we actually mean: "You are killing yourself slowly and I'm watching it happen." Politeness is costing patients their lives.

People's logic: Will research for 3 weeks before buying a phone. Will not spend 10 minutes understanding their cholesterol report. Will drive 2 hours for a "famous" biryani. Will not walk 20 minutes around their own colony. Priorities are not broken. They're inverted.

You don't need: A biohacking protocol Continuous glucose monitors VO2 max testing Expensive longevity supplements You need: A lipid panel once a year HbA1c checked Blood pressure monitored 150 minutes of exercise per week Less processed food Simple. Boring. Effective.

You can think sitting down. But there's something about walking that changes the quality of thought. Ideas arrive differently. Clarity emerges naturally. Like your mind needs your body to move before it can move too. Walking isn't just exercise. It's the way your mind solves itself

"I'm trying Ayurveda for my diabetes." "Homeopathy fixed my uncle's BP." "I'm doing natural healing instead of chemicals." Meanwhile: HbA1c climbing. Blood pressure uncontrolled. Arteries silently narrowing. Alternative medicine feels gentle. Heart attacks are not.

Most patients I see: Obsessed with the money they're making. Not even slightly worried about their health. They'll negotiate a ₹500 discount on a car. But won't spend 30 minutes walking. They're building wealth they won't live to enjoy. The richest man in the ICU is still in the ICU.

Health influencers: Never seen the inside of a cath lab Don't know what apolipoprotein B is Can't explain primary vs secondary prevention Have no idea how atherosclerosis works But they'll confidently tell you to quit your statins. Their business model depends on you distrusting your doctor.

Things that won't fix your diabetes: Detox juice Karela shots "Takat ki dawa" from the chemist Supplements your cousin recommended That YouTube guy's 21-day protocol You know what works? Eating less sugar. Moving more. Taking your metformin. Boring? yes. Effective? 100%

When I tell patients to exercise, they hear "walk." Walking is good. But it's not enough. You don't need to grind in the gym. You don't need to look like a trainer. You don't need fancy goals or tracking apps. Just show up. Stay consistent. That's the only metric that matters.

One day you'll take your last walk. You won't know it's your last. Could be to the bathroom. Could be to the kitchen. Could be to the ambulance. Or could be 30 years from now, holding a grandchild's hand. You choose which last walk. Starting with today's first walk.

Pharma companies could have: Run campaigns on medicine compliance Educated people on heart disease prevention Helped patients understand why they need lifelong medication Instead they: Marketed irrational drug combinations Promoted multivitamins on TV Prevention doesn't sell. Treatment does.

You wait to feel motivated. The motivated feeling never comes. So you wait longer. Years pass this way. Meanwhile, the person who acts without waiting builds an entirely different life. Same amount of time. Different requirement for starting.

Patients in OPD: "Doctor, I don't have time for exercise. Too busy with work." Same patient 6 months later: Admitted for 10 days. Lost a month of work. ₹8 lakh hospital bill. Now has "time" for cardiac rehab. You don't find time for health. You lose time without it.

12-year-old asked his dad in my OPD: "Why do you take so many medicines?" Dad looked at me for answer. I stayed quiet. The silence said everything: "Because I didn't walk when I was supposed to." Kids ask questions we spend lifetime avoiding.

Food has become identity. "I'm pure veg." "Humare yahan to mutton aksar hi banta hai." I'm not asking you to abandon your culture. I'm asking you to not let your culture kill you at 55. Your traditions can adapt. Your arteries cannot.

walking is magic. You leave with one set of thoughts. You return with completely different ones. Something about moving your body moves your mind. Problems that felt impossible before the walk feel solvable after. Like the motion shook something loose.

Our building watchman knows who'll die first. "Sahib takes lift for 1st floor" "That uncle never walks" "Madam only uses driver" He's seen 3 cardiac emergencies. All were lift-users. The poorest person in your building knows your future. Do you?

The 5-year heart patient cycle: 1. Angioplasty done 2. Feels fine 3. Stops medicines 4. Stops testing 5. "Sab theek chal raha tha" 6. New cardiac event 7. New stents 8. Back to step 1 I've watched patients do this 3 times. Some don't get a 4th chance.

Motivation is beautiful when it shows up. But habits are what love you back consistently. They show up on your worst days. They carry you when you can't carry yourself. The aim is not building motivation, but a life that doesn't need it.

People delay coming to the hospital because of ego. "It's just gas." "I'll be fine by morning." "I don't need a doctor." They sit with chest pain for 6 hours before someone forces them in. By then, heart muscle is dead. Permanently. Your ego is not stronger than a heart attack. Stop pretending.

once you build the habit, you free yourself from the exhausting cycle of starting over. No more new year resolutions No more Monday promises. No more "this time will be different." Just steady, reliable progress. That peace is worth more than any burst of motivation.

Most people don't know these numbers: LDL cholesterol: below 100 mg/dL Triglycerides: below 150 mg/dL HbA1c: below 5.7% Blood pressure: below 140/90 They get tests done. They don't know what the results should be. They repeat the cycle blindly. Know your numbers. They decide your future.

The patient is trying to change. Then the family arrives: "Thoda kha le, kya hoga." "Ek ladoo se kuch nahi hota." "Festival hai, chhod de diet aaj." Love in Indian families is often served in sugar and ghee. The patient fights the disease. Sometimes the bigger fight is at the dinner table.

3:30 PM, school pickup: 20 cars with AC on, parents scrolling phones Waiting 30 minutes for gate to open School is 800 meters from most homes. We're raising kids who think walking is poverty. Then wonder why they have diabetes at 25. The cycle you're creating will kill them.
