Nearly 2/3 of Californians are worried about affording unexpected medical bills. More than half worry about out-of-pocket costs and monthly premiums. Health insurance is supposed to protect people from exactly these fears. Instead, for a lot of families, it creates them.

Among California plans reporting to DMHC, the dollars allocated to profit jumped from $399 million in 2022 to $2.9 billion in 2024—a 626% increase. Prescription drugs remained roughly 12% of the premium dollar after rebates. So what is driving premiums higher?

To an insurance company, a denial is just another number on a spreadsheet. To the patient, it means pain, bills and waiting for care their doctor says they need. So who should make medical decisions—your doctor or an insurance company protecting its profits?

Rebates. Spread pricing. Fees. Billions of dollars move through the system, but patients and employers are left guessing who got paid and whether any savings reached them. PBMs are running a shell game.

California regulators assessed $214.3 million against health plans that violated the law in 2025—up from $83.6 million in 2020. That is a 156% increase in five years. Patients are paying more for coverage while health plans are racking up more penalties for breaking the law.

Mark Cuban has spent years digging into America’s health care system. His verdict: opaque, convoluted and dominated by companies focused on getting bigger. Patients don’t need bigger health care conglomerates. They need affordable care they can actually access.
