How many times must we watch our residents and trainees suffer and die before we act? Please check-in with your residents. Don’t accept “fine” as an answer. Residency is an isolating time. Our sleep is poor. We take awful care of our health.

Do financially independent physicians need Long Term Care Insurance? Everything I read and learn tells me no. Why spend (a lot) of money on premiums with a limited benefit? It seems if you are FI then you can afford to self-insure against this risk. What am I missing?

I was talking to a surgical colleague this morning in the physician’s lounge. She wanted to work part-time or at the very least, stop taking overnight call. And she was running into all sorts of roadblocks. So much so, the path forward may be to leave medicine altogether.

For all of the anesthesia residents and fellows applying for jobs, here is the first thing you want to look for in any employer. Are you ready? It’s not compensation. Yes, this is important but not #1. It’s flexibility.

Many fintwit folks prefer a 30 year mortgage for the increased cash flow flexibility. And then claim they can always make *extra* payments to pay off the mortgage faster. I totally understand how you could value the flexibility.

Is a 15 year mortgage always the best choice for you? Or should you pick the 30 year mortgage to have more cash flow flexibility? As with so many things in finance, the choice is *personal* and depends on what you value.

I often talk about skiing with patients. We live in Bend and have a solid mountain here. She said the dream is to ski 100 days a season. And it brought me back to my 4th year of med school. My home resort - Alta - got 702” of snow.

I had no idea what a crazy world navigating nannies, Au Pairs, and childcare in general was before having a child. Yes, great childcare is expensive AND, as a society, caregivers are wildly underpaid and undervalued.

What is the number one hurdle physician’s face in building wealth? Debt. And most commonly, student loan debt. You start with a negative net worth and have to shovel fast to fill in that huge hole.

I just finished an intense five-day, forty-eight hour NOLS Wilderness First Responder training. We had a blast! It was incredible to learn how to use our clinical skill set in remote and austere environments.

Explain to me why I should pay for the COLA rider on my disability insurance. It’s an expensive rider. The increase doesn’t kick in until you have a disability claim. Maybe COLA makes sense early in your medical career. Consider dropping it as you get mid-career or later.

When I take on a new client, most of our discussions revolve around values. Not money. Or even goals. But values. Why? When big decisions come up or life knocks you around, we already know the answer.
