Yes, this is a biologically plausible hypothesis. In AC, the absence of a corpus luteum may affect vasoactive and angiogenic pathways involved in implantation and early placentation. This could contribute to the higher risk of hypertensive disorders and adverse perinatal outcomes observed #ESHREJC

#ESHREjc The CL is far more than a progesterone pump. Beyond P4, it secretes relaxin, VEGF, oestrogen metabolites, prostaglandins, prorenin, and 5α-dihydroprogesterone — none of which are replaced in artificial cycles. Each has a distinct role at the implantation window 👇

Interesting wrinkle #ESHREjc: AC shows more HDP & LGA, but SGA, preterm & birthweight are null — closer to an endometrial-metabolic phenotype than placental insufficiency? However, worth holding lightly given the obstetric data were partly incomplete. Curious how others read it.

Also sperm, yes. It is hypothesized that it is caused by the lack of seminal plasma (in donor semen) and full non-self nature of the embryo (in donor oocytes). Of note, the effect seems cumulative, with double donation risk > oocyte > semen. #ESHREjc
