Environmental health - one of the most underfunded & politically contested areas of public health policy. Why? The benefits are long-term & statistical. The costs are immediate & politically visible. The industries whose emissions cause the damage are powerful voices where decisions are made.

Medicine has always held that information in trust. The digital age has simply made the stakes of that trust more visible and the consequences of its breach more difficult to repair.

Issue #10 | On memory: what we keep, what we lose and what keeps us
Memory is not the past. It is what the past leaves behind when it becomes the present.

The pandemic revealed something else (beyond systems' resilience). The most resilient communities were not always the best-resourced ones. Social cohesion, the degree to which people trusted each other and their institutions and were willing to act collectively, proved as important as hospital beds.

Issue #9 | On resilience: what bends without breaking
The willow bends. The idea survives. The lesson accumulates.

We tend to think in straight lines. Cause, then effect. Before, then after. But the world, most of the time, runs on networks: on the unexpected connections between things that seemed unrelated until suddenly they were not.

Rudolf Virchow said that medicine is politics on a grand scale. Mental health policy, perhaps more than any other, proves his point. The decision to fund it adequately is not primarily a medical decision. But rather one about what kind of suffering a society chooses to take seriously.

History is full of thresholds that looked, at the time, like ordinary days.

Democratic systems struggle to make investments whose returns arrive in twenty years. Planting trees for shade you will never sit in (as the quote) suggests requires a particular kind of civic courage. It is exactly the kind of courage that health policy most needs and most rarely finds.

Issue #6 | On time: what we inherit, what we build, what we leave
We cannot stop time. But we can choose what we plant before it runs out.

What Snow’s map made visible was precisely what medicine had been ignoring: that disease is not only a biological event happening inside a body. It is a geographical and social event embedded in infrastructure, inequality, and the decisions of city planners.

Issue #5 | On invisible things: what we don't see shapes everything
Invisible things are not absent things. They are simply things we have stopped looking at. That, I think, is exactly why they are worth examining.

Reinvention, I have come to believe, is not the opposite of continuity. It is its deepest expression. You only reinvent what you care enough to preserve.

What Germany illustrates & what I find most instructive as a health policy researcher is that UHC & equity are not the same thing. You can guarantee everyone access to care and still produce very unequal experiences of that care. The gap between the two is where most interesting policy work happens.

I have been thinking about borders this week, not the political kind (though those too) but the quieter ones. The border between one language and another. Between who you were in one country and who you become in the next. Between a history that shaped you & a present you are still learning to read.
