Good example of why LV ejection fraction is an overrated measurement. Big left ventricles with a low LVEF can have a reasonable stroke volume. But it doesn’t take much to tip them over the edge.

More than 20% RV apical pacing and I’d want to upgrade to CRT. Similar with frequent PVCs that are amenable to catheter ablation in HF patients.

Important question. We keep SGLT2i going in patients who are supported with inotropes and waiting for an urgent heart transplant. But we tend to stop in those on temporary MCS. I’ve been looking to diagnose euglycaemic ketoacidosis for ages, but just not seen it yet.
