“Just section him at the gate.” If only it were simple. It requires an AMHP, two doctors, in-person assessments and a bed. Meanwhile, services can argue over responsibility until lawful custody ends. We need a national protocol. I’ll run a workshop at the RCPsych QNPMHS Forum on 21 October 2026.

Transfer delays aren’t just a prison healthcare problem. Recent inspections suggest the same issue persists across very different prisons. As 28-day expectations and ICB commissioning approach, we need better evidence on where delays occur if we’re serious about fixing them.

Extreme heat in prisons creates clinical risk. People taking lithium are especially vulnerable because dehydration can raise lithium levels and cause toxicity. Heat plans must ensure access to water, ventilation, staff awareness and timely clinical review. imb.org.uk/news/a-syste...
