Maybe a genjutsu-enhanced mirror or something that shows him a fatter reflection of himself every time he looks into it, and then he gets the compulsion to eat himself to that size. And in his mind, he doesn't realize he's gaining anything, so he still tries to be normal even as he gets bigger.
