I present a corrupted werewolf named Al. Technically more weredog, but he would never admit that. Those extra eyes will move/shift anywhere on his body to keep his specific prey in sight so their positioning is very fluid as is the dead plant growth.

Asong gubat mix werewolf with 4 additional eyes and dead plant matter growing out of his fur.

This is Kay Pasa, he's a nightmare warlock. His pact is with the Boogieman. His spells are all dream or nightmare flavored. He/him. I am an old NB forever DM for a lot of TTRPG campaigns. Kay is my first character in awhile that I've gotten to just play. I love horror and fantasy.
