“The patient's too sick for a vaccine.” Is it true? Data shows that if they're not floridly septic, not on B-cell depleting agents or transplant immunosuppression, seroconversion rates are good. Punt it outpatient and 50–70% never get it. Ep. S5E22. @antibioticdoc.bsky.social

Clinical pearl: measles has one of the highest secondary attack rates of any pathogen — up to 90% in susceptible contacts sharing airspace hours after exposure. It's not “just a fever and a rash.” @antibioticdoc.bsky.social explains why on S5E22.

Clinical pearl: the 2026 Surviving Sepsis guidelines are moving harder against albumin for upfront resuscitation — no consistent mortality benefit, ~100x the cost of crystalloid. Balanced crystalloid (Ringers) now gets a conditional nod over saline. #CriticalCare #FOAMcc #ICU #MedEd

Ebola PPE in a non-climate-controlled facility: the biggest challenge isn't fear. It's heat. Dr. Fowler: if you cover your mucous membranes and avoid sharps injury, you're safe. The donning is easy — the doffing is where the risk lives. S5E19.

With high-resource ICU care (ventilation, RRT, vasopressors), Ebola mortality drops to ~18%. Without it, it's the inverse. Monoclonals cut absolute mortality by 20% in the DRC trial. The Bundibugyo strain? We don't have those yet — trials are starting. #CriticalCare #FOAMcc #ICU

Some of the biggest questions in critical care can’t be answered with an RCT. Pre-hospital trauma intubation is the classic example: the sickest patients get intubated, so observational studies have spent 40 years making intubation look harmful. Full Critical Care Commute episode ↓
