Radiomic features may discriminate outcomes without identifying the image property responsible for that discrimination. We evaluated a patient-specific surrogate framework that tests feature responses to explicitly defined image modifications and diagnoses limitations of the surrogate procedure itself. Four public brain tumor magnetic resonance imaging collections contributed 1699 studies passing initial quality control. Core validation used smaller subsets, including 40 depth-stratified control evaluations from 38 glioma subjects, alongside a constructive counterexample and Gaussian-null diagnostics. Thirteen quantile-based morphological and topological descriptors were compared with conventional radiomics. Binary image pairs with exactly equal distance-1 co-occurrence counts differed in Euler characteristic beyond an independent ensemble’s range in 16/16 replicates, versus 0/16 negative controls. This establishes a specific limitation of those co-occurrence measurements. The Gaussian spectrum estimator was parameter-sensitive: only three of five evaluated settings satisfied an operational negative-control diagnostic, which does not establish statistical calibration. Under the depth-stratified null, the median fraction of descriptors detecting planted objects was 61.5% for the topological set, versus 13.7% and 31.5% for conventional features at 24 and 64 bins. These are descriptive, configuration-dependent comparisons; dependent features, control-based selection, and incomplete spectral convergence limit inference. Secondary clinical applications did not establish biological transferability or prognostic benefit. The framework provides reproducible tests of specified feature–null combinations, while exposing the controls and assumptions needed to interpret them.

Venous and arterial thromboembolic events associated with osimertinib in patients with lung cancer: a study by the thrombosis and cancer group of the spanish society of medical oncology (SEOM) - Clinical and Translational Oncology
Purpose To determine the real-world clinical practice incidence and predictive factors of venous and arterial thromboembolic events (VTE/ATE) in lung cancer patients treated with osimertinib. Methods/patients A multicenter retrospective study including 546 lung cancer patients who initiated treatment with osimertinib before or during 2024, with a minimum follow-up of 6 months. The primary objective was to evaluate the incidence of VTE/ATE. Secondary objectives included identifying predictive factors for thrombosis and assessing its impact on overall survival (OS). Competing risk regression (Fine-Gray), multivariable Cox models, and Kaplan–Meier analyses (including landmark analysis at 6 months) adjusted for time-dependent covariates were performed to prevent immortal time bias. Missing data were handled using a complete-case strategy (n = 544) given <0.5% missingness in model covariates. Results Overall VTE/ATE incidence was 15.0% (82/546; 10.5 events per 100 person-years)[cite: 3, 4]. The cumulative incidence of first VTE/ATE (accounting for non-thrombotic death as a competing risk) was 8.34% (95% CI 6.08–10.74%) at 6 months and 10.54% (95% CI 7.96–13.21%) at 12 months. In the multivariable Fine-Gray model, higher body mass index (BMI) (sHR 1.07; 95% CI 1.02–1.12; p = 0.009) and central nervous system (CNS) metastases (sHR 2.30; 95% CI 1.30–4.07; p = 0.004) were independently associated with VTE/ATE. Metastatic recurrence versus de novo stage IV showed no association with thrombosis (sHR 0.85; 95% CI 0.48–1.50; p = 0.573). Median OS was significantly shorter in patients with VTE/ATE (22.0 vs. 31.0 months; log-rank p = 0.049). Modeled as a time-dependent covariate, thrombosis significantly increased mortality hazard (HR 2.43; 95% CI 1.78–3.33; p < 0.001). Sensitivity analysis excluding events at tumor progression confirmed these associations (Cox HR 1.78; p = 0.002). Neoplastic progression accounted for 77.5% of total deaths and 64.9% of deaths in patients with thrombosis. Conclusions The incidence of thromboembolic events during osimertinib treatment in routine practice was 15.0% and was associated with poorer overall survival. Elevated BMI and CNS metastases identify higher-risk patients, providing a rationale for evaluating individualized risk stratification in future prospective clinical trials.

Real-World Use of Opicapone in Management of Patients with Parkinson’s Disease and Early Motor Fluctuations in Spain: 12-Month Interim Analysis of REONPARK Study
Objective: To evaluate the sustained real-world effectiveness and safety of catechol-O-methyltransferase (COMT) inhibitors added to levodopa in patients with Parkinson’s disease (PD) presenting early motor fluctuations. Methods: REONPARK is an ongoing, multicenter, prospective observational study conducted in Spain. Adult patients with PD and end-of-dose motor fluctuations for <2 years initiating a COMT inhibitor were included. Outcomes assessed at 3, 6, and 12 months included Clinician and Patient Global Impression of Change (CGI-C, PGI-C), Movement Disorder Society–Unified Parkinson’s Disease Rating Scale (MDS-UPDRS), Wearing-off Questionnaire (WOQ-19), Non-Motor Symptoms Scale (NMSS), Parkinson’s Disease Questionnaire-8 (PDQ-8), and safety. This article presents the 3-, 6-, and 12-month planned interim analysis of the study. Results: A total of 143 patients were included (99.3% treated with opicapone), of whom 138, 131, and 89 patients completed the 3, 6, and 12 months of follow-up, respectively. Levodopa doses remained stable in most patients (73% at 12 months). Significant and sustained improvements were observed in MDS-UPDRS Parts III and IV scores, as well as the total MDS-UPDRS score over 12 months, with greater likelihood of Part IV improvement in patients receiving <4.5 mg/kg/day of levodopa. OFF time decreased by 1.6–2.0 h from baseline. ON-time quality was maintained over time with 97.7% of patients reporting no or minimal functional impact from dyskinesia, while 47.7% of patients had no impact from motor fluctuations and 36.4% of patients no longer experienced OFF periods at 12 months. PDQ-8 scores remained stable and no overall benefit on total non-motor symptoms burden was demonstrated, despite improvements in selected domains among patients with a higher baseline non-motor symptoms burden. WOQ-19 scores improved significantly. Conclusions: In PD patients with early motor fluctuations, opicapone initiation was associated with sustained improvements in motor outcomes and reduced OFF time, and maintained the ON-time quality over 12 months, with stable levodopa dosing.

Post-Acute Nasal Expression of ACE2, TMPRSS2, FURIN, and NRP1 in Relation to COVID-19 Severity: A Multicenter Cross-Sectional Study
Background/Objectives: Host factors angiotensin-converting enzyme 2 (ACE2), transmembrane protease-serine 2 (TMPRSS2), furin paired basic amino acid cleaving enzyme (FURIN), and neuropilin-1 (NRP1) facilitate severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) entry, but it is unclear whether their upper-airway expression after recovery is associated with the severity of the preceding acute illness. We examined the association between post-acute expression of these genes and previous coronavirus disease 2019 (COVID-19) severity. Methods: This multicenter cross-sectional study included 104 adults with polymerase chain reaction (PCR)-confirmed COVID-19 during the first two pandemic waves in Murcia, Spain. Nasal and/or oropharyngeal swabs were collected in the post-acute phase, at a median of 75 days after symptom onset, and transcript levels were quantified by quantitative real-time PCR. Severity was categorized using the World Health Organization (WHO) Clinical Progression Scale, and associations were evaluated using logistic regression adjusted for age, sex, and race/ethnicity. Results: In the primary analyses using continuous expression measures, none of the four genes was significantly associated with severity. In exploratory tertile-based analyses, the intermediate ACE2 tertile (odds ratio [OR] = 0.17, 95% confidence interval [CI] 0.05–0.61; p = 0.007) and intermediate NRP1 tertile (OR = 0.29, 95% CI 0.10–0.88; p = 0.030) were associated with lower odds of severe disease; no significant associations were observed for the high tertiles or for FURIN or TMPRSS2. Conclusions: Primary adjusted analyses did not reveal statistically significant associations between post-acute nasal expression of ACE2, TMPRSS2, FURIN, or NRP1 and COVID-19 severity. Because expression was measured after clinical recovery, these associations cannot be interpreted as predictors of acute severity and may instead reflect persistent molecular remodeling after more severe disease; reverse causation cannot be excluded. Longitudinal studies with acute-phase and serial post-acute sampling and healthy controls are needed.

Historical Evolution of Unrelated Donor (URD) Recruitment of the Murcia Region Registry and Conditioning Social Events: Assessment of Quality, Effective URD Improvement, and Molecular HLA Characterization by NGS
The Murcia Region unrelated donor (URD) registry was established in 1992 as part of the Spanish Bone Marrow Donor Registry (REDMO). This study aimed to comprehensively characterize the long-term evolution of URD recruitment, human leukocyte antigen (HLA) genetic diversity, allele and haplotype frequencies, and the spectrum of novel HLA alleles detected over the 34 years from 1992 to 2025. Recruitment trends, URD demographic characteristics, HLA allele and haplotype frequencies, and newly detected HLA alleles were analyzed retrospectively. HLA typing was performed using serological methods (1992–1996); PCR-SSO, PCR-SSP, and PCR-SSO Luminex (1997–2018); sequence-based typing (SBT, 2018–2021); and next-generation sequencing (NGS, 2021–2025). NGS was used to analyze a subset of 650 URDs to determine high-resolution HLA allele frequencies, Hardy–Weinberg equilibrium, expected heterozygosity, and extended HLA haplotypes. Newly detected HLA alleles were further characterized by the affected HLA locus and the localization of sequence variants within exons, introns, and the 3′-untranslated region (3′UTR). Between 1992 and 2025, 33.271 URDs were recruited, of whom 27.107 remained active in REDMO at the time of analysis. URD recruitment increased substantially following the implementation of the Spanish National Bone Marrow Plan in 2012 and reached a historical high in 2017. Additional recruitment peaks were associated with major regional public awareness campaigns. High-resolution HLA analysis of the NGS cohort demonstrated extensive immunogenetic diversity across all analyzed HLA loci, with a mean expected heterozygosity of 0.914. Ten frequent extended HLA haplotypes were identified, the most prevalent being HLA-A*29:02-C*16:01-B*44:03-DRB1*07:01-DQB1*02:02. Thirty-nine novel HLA alleles were characterized, exhibiting heterogeneous distributions among HLA loci and across coding and non-coding genomic regions. Exonic variants were predominantly detected in HLA-C, whereas intronic variants were mainly observed in HLA-C, HLA-DQB1, and HLA-A. This study integrates 34 years of URD recruitment with high-resolution immunogenetic characterization of the Murcia Region URD registry, providing new insights into the evolution of donor recruitment, HLA diversity, and the discovery of novel HLA alleles. The implementation of NGS enabled precise molecular characterization of HLA diversity and substantially expanded the detection and characterization of novel HLA alleles. Together, these findings provide a robust immunogenetic framework for the Murcia URD population, supporting donor recruitment strategies, optimization of URD searches, registry management, and future research in population genetics and hematopoietic stem cell transplantation.

Background/Objectives: Oseltamivir is a widely used antiviral agent indicated for the treatment of influenza and plays a central role in pandemic preparedness strategies. Although an extemporaneously prepared 15 mg/mL oral solution is recognized in formularies, preparing large volumes during a pandemic requires extended storage options. However, no stability studies are currently available for an oseltamivir 15 mg/mL oral solution stored under freezing and subsequent refrigerated conditions for the duration of a standard treatment. The aim of this study was to evaluate the physicochemical and microbiological stability of a 15 mg/mL oseltamivir oral solution prepared from the active pharmaceutical ingredient (API) and packaged in amber glass containers. Methods: The oral solution was formulated using oseltamivir phosphate API, sodium benzoate as a preservative, and purified water and then packaged in 125 mL Type II amber glass bottles, allowing for thermal expansion. The samples were stored at −20 ± 2 °C for up to 75 days, followed by refrigerated storage (5 ± 3 °C) after thawing for up to 10 days. Chemical stability was assessed using a validated HPLC method in accordance with ICH guidelines and was defined as 90–110% recovery of the initial concentration. Physical stability (color, pH, particulate matter, crystallization, and homogeneity) and microbiological stability were also evaluated. Results: The HPLC method demonstrated excellent linearity, precision, and accuracy. Oseltamivir concentrations remained within the predefined acceptance limits throughout the 75-day study period under freezing conditions, with no significant changes in pH, color, or particulate formation. After thawing, the drug concentration continued to remain fully stable and within the required limits for up to 10 days under refrigerated conditions. Despite the prolonged storage and phase changes, no significant changes in physical parameters were observed, and microbiological testing confirmed the absence of aerobic, anaerobic, and fungal microorganisms on the final day of the study. Conclusions: Oseltamivir 15 mg/mL oral solution in amber glass bottles is physicochemically and microbiologically stable for up to 85 days (75 days under frozen conditions plus 10 days under refrigeration after thawing).

Burnout is recognized in the ICD-11 as an occupational phenomenon resulting from chronic work-related stress. In educational settings, psychosocial risks—particularly high job demands and insufficient resources—have been associated with burnout, depressive symptoms, and physiological stress dysregulation. This longitudinal quasi-experimental study examined changes associated with participation in a multicomponent cognitive–behavioral intervention among teachers. The sample comprised 66 primary and secondary school teachers (30 training, 36 non-training), further stratified according to high or low baseline levels of stress-related health problems. Measures included resource availability, self-efficacy, interpersonal conflicts, work overload, burnout, depressive symptoms, and salivary cortisol, assessed as total morning cortisol output using the area under the curve with respect to ground (AUCg), at baseline (T1), post-intervention (T2), and four-month follow-up (T3). The clearest pattern of statistically significant within-group changes was observed among trained teachers with high baseline stress-related health problems (TG-HHP; n = 13). From T1 to T2, this subgroup showed increased resource availability and self-efficacy and reductions in burnout (1.63 to 1.33; p = 0.013, d = 0.81), depressive symptoms (46.92 to 42.40; p = 0.032, d = 0.67), and morning cortisol AUCg (410.01 to 306.50; p = 0.018, d = 0.78). At T3, statistically significant changes relative to baseline were observed across all study variables. These findings provide preliminary evidence that participation in a structured cognitive–behavioral intervention may be associated with psychosocial, psychological, and physiological improvements, particularly among teachers with elevated baseline stress-related health problems.

Purpose To evaluate whether early vascular susceptibility, reflected by first-trimester lipoprotein(a), together with longitudinal renal biomarker trajectories across pregnancy, is associated with the subsequent development of preeclampsia. Methods Retrospective analysis of prospectively collected data from a high-risk pregnancy cohort. Maternal serum inflammatory (interleukin-6, C-reactive protein), lipid (lipoprotein(a), triglycerides, total cholesterol), and renal (uric acid, cystatin C) biomarkers were measured at 11–13, 19–22, and 32 weeks of gestation. Longitudinal changes were assessed using population-averaged models including trimester, preeclampsia status, and their interaction, adjusted for maternal age and body mass index. First-trimester lipoprotein(a) was evaluated using adjusted logistic regression. Results Among 119 women, 20 developed preeclampsia. First-trimester lipoprotein(a) concentrations were higher in women who later developed preeclampsia and were independently associated with increased risk (OR 2.38; p < 0.001). Longitudinal analyses showed significant trimester-by-preeclampsia interactions for uric acid and cystatin C (both p = 0.03), indicating progressive divergence across pregnancy. Inflammatory biomarkers showed early between-group differences without significant longitudinal divergence. Conclusions Our findings suggest that preeclampsia may be associated with both early maternal vascular susceptibility and progressive alterations in renal biomarkers during pregnancy. Elevated first-trimester lipoprotein(a) may reflect an underlying vascular phenotype, while divergent renal biomarker trajectories highlight the dynamic evolution of the disease before clinical onset, although these findings require validation in larger prospective studies.

A New Gain-of-Function NLRP1 Variant Is Associated with Inflammasome Activation and Disease Severity in SLC25A38 Congenital Sideroblastic Anemia Case Report
Inflammasomes play a pivotal role in inflammation and infection, but the clinical relevance of inflammasomes goes beyond infectious disease. Here, we report a novel variant in the NLRP1 gene (p.V939M) identified in a patient with congenital sideroblastic anemia due to a homozygous SLC25A38 mutation. This patient exhibits severe anemia, iron overload, and dependence on frequent transfusions. Functional characterization of peripheral blood mononuclear cells from the patient revealed constitutive activation of the NLRP1 inflammasome in primary cells, evidenced by increased processing of NLRP1, cleavage of CASP1, and ASC oligomerization. Moreover, in a speck in vitro assay, the newly identified genetic variant increases ASC oligomerization, and using ex vivo erythroid differentiation assays with patient-derived cells, we demonstrated that pharmacological inhibition of CASP1 enhances erythrocyte formation. This patient-derived NLRP1 variant was evaluated in a newly generated human NLRP1 knockdown system using CRISPR-Cas9 in K562 cells, in combination with ferroptotic/oxidative stress induction to mimic patient clinical features, and our results indicate that the presence of NLRP1 p.V939M, together with enhanced ferroptosis, may contribute to the patient’s phenotype. Furthermore, both CASP1 inhibition and ferroptosis inhibition further enhance the restoration of erythroid colony formation from primary progenitor cells derived from the patient. Therefore, our study identifies a novel NLRP1 inflammasome variant involved in human erythropoiesis and highlights its possible contribution to the pathogenesis of congenital sideroblastic anemia, providing a basis for the development of new potential therapeutic strategies.

Longitudinal Changes in Illness Acceptance, Psychological Adjustment, and Quality of Life After Robot-Assisted Radical Prostatectomy
Background: Prostate cancer is one of the most commonly diagnosed cancers in men. Robot-assisted radical prostatectomy (RARP) is one of the recommended surgical treatment options; however, knowledge regarding changes in illness acceptance, psychological adjustment, and quality of life after surgery remains limited. Objective: To evaluate changes in illness acceptance, psychological adjustment, and quality of life in patients undergoing RARP. Materials and Methods: A total of 150 patients diagnosed with prostate cancer and scheduled for RARP were enrolled. Longitudinal analysis was performed in 93 patients who completed both study assessments. Evaluations were conducted before surgery and 3–4 months postoperatively using a self-developed questionnaire, the Acceptance of Illness Scale (AIS), the Mini-Mental Adjustment to Cancer (Mini-MAC), the European Organisation for Research and Treatment of Cancer Quality of Life Questionnaire Core 30 (EORTC QLQ-C30), and the prostate cancer-specific module (EORTC QLQ-PR25). Results: Following RARP, illness acceptance did not change significantly after Holm adjustment (pHolm = 0.465). Psychological adjustment showed an adverse postoperative pattern, characterised by increased anxious preoccupation, helplessness–hopelessness, and destructive style, together with reduced fighting spirit and constructive style. After adjustment across 25 paired outcome comparisons, significant increases remained in emotional and cognitive functioning scores, while constipation and diarrhoea decreased significantly. Sexual activity also decreased significantly after surgery. Among participants who completed both assessments, incontinence-aid use increased from 15/93 (16.1%) before surgery to 38/93 (40.9%) at follow-up (exact two-sided McNemar test, p < 0.001). Conclusions: During the early postoperative period following RARP, favourable changes were observed in selected quality-of-life scores, whereas illness acceptance did not change significantly after adjustment for multiple comparisons. At the same time, psychological adjustment showed an adverse pattern, characterised by reduced fighting spirit and constructive style together with increased anxious preoccupation, helplessness–hopelessness, and destructive style. Decreased sexual activity and increased incontinence-aid use further illustrate the multidimensional nature of early postoperative recovery. These exploratory findings indicate that favourable changes in selected quality-of-life outcomes do not necessarily coincide with improved psychological adaptation and support the inclusion of psycho-oncological assessment and support in postoperative care.

Acute Physiological Response Variability in Lidia Bulls Following Roping Bull Celebrations: Effects of Age, Previous Participation, and Sampling Period
Roping Bull celebrations involve prolonged physical activity in Lidia cattle and are associated with marked acute hematological, biochemical, and endocrine responses. Previous research has characterized these physiological changes, but less is known about the factors that may explain inter-individual variability in their magnitude. The present study evaluated whether age, previous participation history, and sampling period influence the PRE–POST physiological response of Lidia bulls. A total of 101 adult Lidia bulls were evaluated immediately before and after participation in a standardized 45 min Roping Bull event. Fifty-three bulls (52.4%) had also been included in a previous study in which the physiological variables evaluated in the present investigation had been reported. Linear mixed-effects models were fitted separately for each biomarker, with Time (PRE vs. POST), age group (AGE), previous participation history (RACES), and sampling period (MOMENT) included as fixed effects, together with prespecified interactions with Time. Individual bull was included as a random intercept to account for the repeated PRE–POST measurements. Participation was associated with significant changes in several hematological, biochemical, and endocrine biomarkers, particularly lactate, creatine kinase, lactate dehydrogenase, leukocyte variables, cortisol, and ACTH. In contrast, AGE, RACES, and MOMENT showed comparatively limited influence on the magnitude of most PRE–POST responses. Selected Time × factor interactions indicated some biomarker-specific inter-individual variability, although these effects were less consistent than the overall Time effect. The present study extends the previous physiological characterization of Lidia bulls by identifying potential factors associated with variability in the acute PRE–POST response. Overall, the physiological response was relatively consistent across age and previous participation categories, although selected interactions indicate that individual and event-related factors may contribute to variation in specific biomarkers. These physiological measurements should not be interpreted in isolation as direct evidence of either preserved or compromised animal welfare.
