Opioid-induced hyperalgesia is likely a driving force for opioid seeking during the first 3 weeks, but only in SOME individuals, those with the most severe addiction-like behavior.

We followed pain for 3 weeks of abstinence after oxycodone IVSA. Rats with mild addiction-like behavior recovered in a few days, like saline controls. Rats with severe addiction-like behavior were still hurting at 3 weeks.

A few years ago Yavin Shaham and Marina Wolf asked me if we had ever tracked pain longitudinally in our addiction-like behavioral model in HS rats. I was shocked and even ashamed that we had not, so we did it ;-) then took a while to replicate data and publish.
Figure 4 from Plasil et al. 2026: mechanical pain thresholds across three weeks of abstinence in naive, saline, Low Addiction Index and High Addiction Index rats. Only High Addiction Index rats show allodynia persisting through three weeks.

is challenged While there are lots of papers in human that carefully describe the development of tolerance during exposure to opioids, the literature is relatively thin on the recovery of pain sensitivity and tolerance during abstinence and the sensitivity to MOR modulators.

Reminds me of the classic work by the Bordeaux crew (Le Moal/Simonnet/Stinus) that showed that there is no real return to baseline during opioid abstinence and that it is more the emergence of a compensated state with sensitization of a pronociceptive system that become visible only when the system

Heroin vapor inhalation in female rats produced almost full tolerance to the analgesic effect opioids in 5-10 days. While there pain (thermal) sensitivity recovered after a month there were long lasting masked effect (15 weeks) on pain sensitivity that were revealed with acute naloxone/heroin.

the good news is they find that buprenorphine and naltrexone can decrease overdose/death up to a year after birth, as long as the medications are being prescribed, and the longest the better. Unfortunatly there are still large stigma and hurdles to get these medications, and only 1/4 gets them.

The first 2 sentences of this paper are brutal. "Unintentional drug overdose is the leading cause of maternal death in the US. Opioid use disorder (OUD) during pregnancy has increased more than 7-fold over the past 2 decades."

This may explain why cannabis at high dose can trigger anxiety in some. I'm curious whether it would also amplifies representation of interoceptive signals which would then also facilitate panic attacks.

This was only in healthy controls with no past drug use. I'd be interested to see what it looks like in people with a substance or alcohol or tobacco use disorder. There is data in rodents showing it may be very different. www.nature.com/articles/s4...

Another surprise, tobacco ranked above alcohol. I didn't have that on my bingo card. The regions the most affected where the prefrontal cortex, insula, cingulate, putamen and thalamus. Right on the limbic and associative cortico-striatal-thalamo-cortical loop!

What's crazy is that this accelerated aging haoppened despite the addiction patients being on average 40 years younger than the Alzheimer's group. let that sink in.

They used MRI of 45,900 healthy people and 2,698 patients and found large effect with brain-age gaps vs controls (Cohen's d) of: Alzheimer's 0.97, alcohol + tobacco 0.84, tobacco 0.72, alcohol 0.62, schizophrenia 0.53, bipolar 0.46, depression 0.28, autism and ADHD near zero.

The main feature lacking though is integration of a proper LLM-poewred search engine in addition of the regular Pubmed Search function and I still don't understand why you can't sort publication results in Pubmed by other metrics than author, date, journal.

So maybe some refining left to do ;-) The feature I would actually use is uploading a draft manuscript or abstract and get the same neighborhood view. That would catch the relevant literature you missed before reviewer 2 does.

PubMed got a new toy today. I tried it on two of our lab's PNAS papers. The visualizations are pretty, it's nice to see PubMed getting upgrades. But I have never found citation graphs like these particularly useful for actual research or writing.

Sobering piece of data. 23% of people with a substance use disorder had a suicidal event in their lifetime. That is 16x the rate without SUD, and ~6x after taking into account many other factors like comorbidities.
