This is very wonky and technical, but the end result is important. If this became law, it would force MedPAC to produce a report that makes Medicare Advantage look *way* better. It would nuke independent research and make it more likely for insurers to keep their MA gravy train.

Worked with @bobjherman.bsky.social on the data for this piece. Here’s the chart I think is the showstopper: Over the last 40 years, the amount of money spent on employer-sponsored insurance premiums skyrocketed by nearly 1000%, well above baseline inflation.

By analyzing pharmaceutical company revenue data with federal health spending data, STAT found the two drugs specifically indicated for weight loss — Novo Nordisk’s Wegovy and Eli Lilly’s Zepbound — accounted for 4% of the $519 billion in retail prescription drug spending in 2025. When combining the revenue for Novo’s and Lilly’s GLP-1 drugs for all conditions, including diabetes, this single class made up 11% of retail drug spending last year.

The selling point of catastrophic plans is their cheaper premiums, but the tradeoff is that they require members to pay for everything beyond preventive care until they hit their deductibles, which CMS proposes to increase to $17,500 for an individual and $35,000 for a family in 2028, according to a report from Veda Partners, an investment adviser. Veda said the individual deductible is roughly equal to the total cost of an average inpatient hospital stay, which lasts 5.5 days and costs $3,300 per day.
