Abstract

Prostate cancer is the most common type of cancer in men but there is no national screening programme in the UK or in most other countries. The UK National Screening Committee (UK NSC) is the independent advisory body responsible for providing evidence-based recommendations to ministers in the four UK nations regarding screening programmes. Following the expansion of its remit to include targeted screening (2022) and the publication of new long-term trial data, the UK NSC launched a comprehensive review of prostate cancer screening in 2025. To inform this review, a bespoke economic model was commissioned, drawing on best available evidence and input from clinicians, technical experts, and patient representatives. The model evaluated whole-population screening and targeted strategies for men at higher risk. Based on the modelling study and consultation with stakeholders, the UK NSC recommended against whole-population screening and screening in most targeted groups. The UK NSC has recommended screening for men with a pathogenic BRCA2 variant with a family history of breast, ovarian, pancreatic, or prostate cancer. This was the only group where the UK NSC had confidence that screening would do more good than harm, primarily because prostate cancer tends to be more aggressive in this group. This narrative review describes the process taken by the UK NSC in developing its recommendations and highlights where additional evidence is still needed.

Objective
To synthesise current international guidance on lutetium-177-labelled prostate-specific membrane antigen (177Lu-PSMA) radioligand therapy (RLT) across the prostate cancer disease spectrum, with emphasis on metastatic castration-resistant prostate cancer (mCRPC) and emerging earlier-line indications.
Methods
A structured literature overview was conducted. PubMed, EMBASE, and official repositories of major urological, oncological, and nuclear medicine societies were searched from inception to 31 October 2025 for prostate cancer guidelines. National and international guidelines or consensus statements published in English were eligible if they provided explicit recommendations regarding 177Lu-PSMA-RLT in their most recent version. Documents without therapeutic recommendations or superseded by updated versions were excluded. Six major society guidelines were included: European Association of Urology (EAU 2025), American Urological Association/Society of Urologic Oncology (AUA/SUO 2023), National Comprehensive Cancer Network (NCCN 2025), Canadian Urological Association (CUA 2024), American Society of Clinical Oncology (ASCO 2025), and European Society for Medical Oncology (ESMO 2020). The joint European Association of Nuclear Medicine/Society of Nuclear Medicine and Molecular Imaging (EANM/SNMMI 2023) procedural guideline and expert consensus from the Advanced Prostate Cancer Consensus Conference (APCCC 2024) were also reviewed.
Results
All societies except ESMO, which has not yet updated its guidance on PSMA-RLT, endorse 177Lu-PSMA-617 for PSMA-positive mCRPC following prior androgen receptor pathway inhibitor (ARPI) and taxane chemotherapy, supported by Level 1 evidence from the VISION and TheraP trials. Following PSMAfore, NCCN version1.2025, and more cautiously the EAU 2025 guideline, acknowledge expansion into taxane-naïve patients progressing after ARPI therapy. Perspectives on technology: All STEPS count – an integrated framework for net zero urological care

Objective
To present a narrative review of evidence to guide the delivery of high-quality, low-carbon urological care using a
structured framework.
Methods
Academic and policy papers which outline actions focused on decarbonising urological care and surgical care more broadly
were identified and reviewed. The ‘STEPS to Low-Carbon Care’ framework (an acronym for low-carbon care across
‘Settings and Treatments, Efficiency, Prevention and System change’) was then used to categorise and present the
evidence-based decarbonisation actions, using the National Health Service in England as a case study.
Results
Across all STEPS framework elements, tangible actions were identified alongside opportunities for future research and
innovation. The evidence-based actions that were identified to transition to low-carbon care settings and treatments
included tackling known carbon hotspots in operating theatres: anaesthetic gases, consumables and electricity use. Outside
the operating theatre, urology pathway transformation through one-stop clinics, day-case surgery, appropriate use of virtual
appointments and streamlined pathways demonstrated opportunities to reduce carbon emissions, with potential additional
benefits in terms of cost, efficiencies, and patient outcome improvements. Key climate mitigation actions that support
keeping people healthy were identified. There was a paucity of evidence demonstrating the implementation of climate
change action as part of routine service delivery. Embedding sustainability across organisational processes and ways of
working requires actions to upskill, engage and enable the workforce to deliver and to establish clinical leadership.
Conclusion
This review identified a range of interventions to decarbonise urological care, whilst highlighting a need for further
