*Addendum for clarification: Current guidelines emphasize shared decision-making when deciding on a rate vs rhythm. Rate control remains an appropriate strategy for permanent AFib, while early rhythm control can reduce adverse CV outcomes particularly in patients with recently diagnosed AFib. 🙏

This is my approach to vasopressors in septic shock in 60 seconds! While general guideline recommendations are useful and followed by most clinicians in most cases, there are opportunities to individualize pressor selection in some patients, especially once you get to that 3rd pressor (or beyond)!

The more QT prolonging medications a patient receives (and the more risk factors they have, like hypokalemia, hypomagnesemia, or bradycardia) the greater the risk of Torsade de pointes. So be careful of concomitant QT prolonging medications from administration!

Additional things to note: QT interval varies with heart rate (HR). A QT interval can be used if the HR is <60 bpm; when the HR is >60 bpm, a QT interval corrected for heart rate (QTc) is used.
