To prevent unnecessary hospital admissions and deliver sufficient support at home to enable hospital discharge towards the end-of-life, investment in and expansion of community-based care is needed. Good work by @assemmkhamis.bsky.social at #EAPC2026

Pain was more common in the most deprived groups, but improvement was similar across all quintiles. In contrast, patients in the most deprived areas were less likely to improve in breathlessness, highlighting a "health gap" in specialist palliative care.
