H1-B workers mitigate the shortage of doctors and other medical practitioners in rural areas. OTOH, Musk fired skilled workers at his companies while applying for H1-B visas to get cheaper, more compliant workers. Medical or otherwise, H1-B workers should all be paid local prevailing salary/wages.

I work for a university and handle the HR tasks for my department. You can transfer an H-1B visa when you switch jobs. There are a lot of hoops to jump through, but it’s all doable (and easier than applying for a new H-1B visa).

My 2 cents? If we really wanted to, we could get Americans those jobs by educating them and driving this investment and fund it, and do you know why we don't? Because lower labor and a workforce from another country will work under different circumstances and not question it.

Don’t miss the forest for the trees - H-1B Visas are routinely used to displace American tech workers who clearly have the skill set to do the jobs taken by the H-1B job seekers. This depresses wages for 10s of thousands of US citizens who have gotten educated and worked hard to build careers.

The sad part is that people don't have a clue about the paperwork, frame time, requirements, and fees that workers need to comply with to get a 3 year valid visa that doesn't allow him/her to switch employer.
