I feel quite strongly that the development and release of 4o needs to be investigated and taught in the same way that Therac-25 is taught in engineering schools.
It was an immense failure of engineering ethics and should be a black mark on every person involved.
@rincewind.runit really is fascinating, it’s not like the problem never happens with other versions but there was a while there where it was like, is this all LLMs or just 4o and it turns out the answer was “4o specifically destroyed people’s minds at a massively disproportionate rate”
